Application Instructions
Step 1 Obtain Student W ID.
Go to www.chabotcollege.edu, on the far right, under “Announcements” APPLY TO CHABOT ONLINE.
Once your application has been processed you will receive a confirmation email with your Chabot W ID
number.
If you do not receive this email, you can use your social security number (as long as it was provided on
your admission application) to look up your student W ID number in CLASS-Web. Follow instructions in
step 2 to log into CLASS-Web. Once you have logged in, click on What is my W ID link.
STEP 2 Navigate to the Dental Hygiene Program Online Application.
Go to www.chabotcollege.edu

Select the CLASS-WEB icon at the bottom of the page.

Next page, select Sign on to CLASS-WEB.

Enter your USER ID (Student W ID number or your social security number)
Enter your PIN. The default PIN is set to your date of birth in the following format: MMDDYY.
Click the Login icon.

Select Student Services and Financial Aid Tab.
Select Dental Hygiene Application Menu from list.
STEP 3: Complete the Dental Hygiene Program Online Application.

You must be able to PRINT to complete the application
If you mistakenly select any of the other menu options, use the BACK arrow to return to this
page.
BEFORE you begin, have your current transcripts available for reference.
You will be required to enter EXACT information in regards to your academic history (i.e., dates
of attendance and specific course names, numbers, grades and the semester/quarter in which
theses course were completed). Inaccurate information will disqualify your application.
STEP 4: Select the Dental Hygiene Application Link.

Select Fall term
Select Chabot College
Select the DENTAL application
Click Submit Query
Read the Notice to Applicant
Click Begin
Proceed through application until you get to the final submission page.
STEP 5: Submit Application

To submit the application, type your name in the acknowledgement box.
Click on Submit Application

You may SUBMIT this application only once. Changes or edits will not be accepted after
submission.
STEP 6: Print

PRINT two copies, SIGN and DATE the CONFIRMATION PAGE.
Submit one copy with your application packet.
Follow the instructions to complete the application process.
An application submitted without this page will be disqualified.
Submit your application packet with all required documentation by the deadline noted.
Mail the following materials US Mail, UPS or Federal Express on or before January 29, 2016.
Your Application Packet must have the following documentation.
1. Dental Hygiene Program Check List
2. Signed and dated CONFIRMATION page
3. Official College Transcripts. (One copy of all transcripts of ALL colleges attended, except
Chabot or Las Positas, must be included with this application.
4. Verification of Residence
It is recommended you make copies of all materials submitted and send US Mail certified with return
receipt. Hand carried application packets will not be accepted. Saturday deliveries will not be accepted.
