Purpose
1.1
To inform the board of progress made by the System Safety Risk Group
(SSRG) during its first year of operation and to secure approval of objectives
for 2014/15.

To demonstrate that the objectives meet the requirements of the board.

Governance
2.1
SSRG was established in 2013 as part of the modernisation of safety
cooperation project (link to project overview) with a remit from the board of
RSSB to understand 100% of system safety risk, identify areas for
improvement, facilitate safety cooperation across the industry including
sharing good practice and horizon scanning to identify potential threats and
opportunities.
2.2
During 2013 SSRG met at eight weekly intervals. Four meetings are planned
for 2014 but in addition risk review workshop sessions are to be organised.
Meetings have been held at RSSB, ATOC and Network Rail to illustrate this is
a body with pan-industry buy-in. The December 2014 meeting will be held at
the ORR offices, London.
2.3
The meeting is chaired by Andy Cooper, MD of Crosscountry Trains, who is
supported by a deputy, Allan Spence of Network Rail. A full list of members
can be viewed on the RSSB website.
2.4
Throughout the year, SSRG has developed its terms of reference and these
received final approval from the RSSB board in November. The terms of
reference are underpinned by six key objectives.

Year 1 progress and planned objectives for 2014/15
3.1
Early SSRG meetings focussed on the development and agreement of six key
objectives aimed at delivering the purpose agreed in the SSRG terms of
reference. The six key objectives are:

3.2
Each SSRG meeting is structured around these six key objectives and
progress is monitored using the RAG process.
3.3
The status of delivery progress against the 2013/14 objectives is summarised
in Table 1 below.
3.4
Table 1 also identifies planned objectives including success criteria for
2014/15. The board are invited to note that these objectives include the
requirement to produce the workforce, passenger and public risk review
papers plus the annual risk review paper as directed by the board and these
papers will be circulated to board members as requested.
3.5
The objectives also include the requirement for SSRG to develop the 10 year
SPAD risk control strategy as requested by the board.
3.6
At this stage the objectives do not include any actions generated by the
Industry Safety Meeting (ISM) on 17 April 2014 but the chair of SSRG is
aware that further enhancements may be required.

3.8
The board are requested to review the planned 2014/15 objectives along with
the output of the recent ISM and determine if further action is required by
SSRG.
4.
Definition of system safety
4.1
SSRG has identified that there is a need to help the whole industry
understand what is meant by system safety, define its scope and clarify
roles/responsibilities through the production of simple visual guidance. (see
also board paper 1).
4.2
Under SSRG objective 1: ‘Understand 100% of system safety risk’, SSRG are
overseeing the work of the System Safety Working Group to develop simple
guidance on the application of best practice in managing the overall risk
drawn from best practice from the rail industry and other high risk industries.
This is proposed as a future agenda item for the board later in 2014.
5.
Recommendations
5.1
The RSSB board is asked to:
a) NOTE the progress made by SSRG against its objectives during the first
year of operation.
b) DISCUSS and APPROVE its planned objectives for 2014/15 taking into
account the output from the recent ISM.
