Fatalities
There were no passenger or workforce accidental fatalities during October 2015. There were 4 public
accidental fatalities:
On 2 October, a person gained access to the track and fell onto a live third rail, resulting in electrocution
near Waterloo East (South Eastern).
On 15 October, a person was struck fatally by a train at Milnsbridge Viaduct (London North Eastern).
On 19 October, a person with injuries consistent with being struck by a train was found on the down line
near Wood Ditton AHBC (London North Eastern).
On 29 October, a person with injuries consistent with being struck by a train was found on the down line at
Feltham (South Eastern).
There were 19 suspected suicides during October 2015. The average monthly figure over the past 12
months has been 22.3. Suicide figures are subject to change as more information (such as coroners'
verdicts) is made available.
Reportable train accidents: collisions, derailments and trains striking road vehicles
On 3 October, a passenger train which was parked at Leeds (London North Eastern) rolled back and
collided at slow speed with another train which was already in the platform during permissive working.
There were no injuries reported.
On 20 October, a track maintance train became derailed on the points at the junction connecting the Mid
Norfolk Railway and the down line at Wymondham (South Eastern). There were no reported injuries.
Precursors
During October 2015, there were 24 signals passed at danger (SPADs). This is 2 fewer than in October
2014. The average monthly figure over the past 12 months has been 24.8.
Of the 24 SPADs in October 2015, 10 were risk-ranked potentially significant (16-19), and no SPADs were
risk-ranked as potentially severe (20+).
In addition to the 24 SPADs, there were 80 other incidents where signals were passed at red (SPARs) in
October 2015; there were 123 such incidents the previous October. Of these 80 SPARs, 71 were due to
signalling system reversion or replacement (system or human), and 9 were due to replacement of the
signal in an operational incident. There were no train runaway incidents.
Up to 14 October 2015 (the period end date), there were no broken rails reported. In September 2015
there were 9 broken rails reported.
