2016 FIRM/ACCOUNTING STUDENT SCHOLARSHIP APPLICATION REQUIREMENTS 1. Each award-­‐winning accounting student MUST plan to be enrolled in an accounting degree-­‐granting program for Fall Semester 2016 with a minimum enrollment of six credit hours. If you are graduating in Summer 2016, you may use the scholarship money for Summer 2016 classes. The enrollment requirement does not apply to students applying for a CPA Review scholarship. 2. Each award-­‐winning accounting student MUST attend the 2016 Spring Awards Banquet, to be held on Friday, April 29, 2016 or you will forfeit the monetary award. 3. Each award-­‐winning accounting student MUST present to the Department of Accounting a copy of a signed “Thank You” letter that will be sent to the scholarship benefactor. This letter must be presented to the Accounting Department no later than May 9, 2016 or the student will forfeit the award. Please fill out form on reverse side. 4. Monte Ahuja College of Business Scholarship Consent to Share Information Release Statement The Family Educational Rights and Privacy Act (FERPA) require student consent to disclose personal and educational information to any individual, which includes scholarship donors. All Monte Ahuja College of Business undergraduate and graduate students who accept scholarship opportunities from the Monte Ahuja College of Business are asked to review our "scholarship consent information release statement" regarding the release of the scholarship applicant's confidential information to a scholarship donor(s). This confidential information includes the following: student’s name, intended major, academic standing, current GPA, possible graduation date, hometown and scholarship essay. Note: All the confidential information is listed on the original scholarship application completed by the scholarship applicant -­‐ you. In addition, on rare occasions the Monte Ahuja College of Business may request to release publication of your name and photograph along with the scholarship award name and information related to scholarship requirements; however, you will be contacted to provide a photo if this request is needed. You can reject this request if requested in the future. For example, six months after your scholarship has been applied to your CSU student account, if the College makes this request to publish your photo and name then you can simply reject this request.
