investigate an outbreak of food borne illness, and advised consumers to avoid eating any
alfalfa sprouts produced by Jack & The Green Sprouts February 25 following seven cases

in the West Palm Beach sewage plant lot and claiming that he had placed bombs at both the

1. March 1, Associated Press – (North Dakota) 2,000 gallons of saltwater, oil spill in
Billings County. The North Dakota Department of Health reported a spill of
approximately 2,000 gallons of saltwater and oil from a pipeline at a White Rock Oil
and Gas-owned site in Billings County February 26. An unknown amount of the
saltwater and oil spilled into a stock pond and officials continue to investigate.

2. February 29, U.S. Department of Labor – (Ohio) OSHA cites auto parts maker for
forklift, machine hazards at Oregon, Ohio, plants after complaints of unsafe
working conditions. The Occupational Safety and Health Administration cited
Oregon, Ohio-based Autoneum North America Inc., for 1 repeat and 5 serious safety
violations February 25 after complaints of unsafe working conditions prompted
investigations at 2 Ohio, Oregon facilities which revealed that the company failed to
develop and train workers on machine specific procedures, operated forklifts
improperly, and failed to train forklift operators, among other violations. Proposed
penalties total $59,500.

Defense Industrial Base Sector
3. February 29, U.S. Department of Justice – (Kentucky) Lockheed Martin agrees to
pay $5 million to settle alleged violations of the False Claims Act and the Resource
Conservation and Recovery Act. The U.S. Department of Justice announced February
29 that Lockheed Martin Corporation and its subsidiaries agreed to pay $5 million to
resolve allegations that the company violated the Resource Conservation and Recovery
Act by failing to identify, report, and dispose of hazardous waste produced and stored
at the Paducah Gaseous Diffusion Plant in Kentucky, while also knowingly submitting
false claims for payment under contracts with the U.S. Department of Energy. The
company was responsible for the facility’s uranium enrichment operations,
environmental restoration, waste management, and custodial care of the facility from

1984 – 1999.

4. March 1, Florida Today – (Florida) Fatal crash investigation ongoing. All
southbound lanes of Interstate 95 near the Pineda Causeway exit in Melbourne were
shut down for several hours February 29 while the Florida Highway Patrol investigated
the scene of a crash.

5. February 29, WTKR 3 Norfolk – (Virginia) 2 dead, 21 boats destroyed in massive fire
at Urbanna marina. A massive fire at the Dozier Yachting Center in Urbanna
February 29 left 2 people dead and destroyed the boathouse, a storage facility, 21 boats,
and 2 parked cars. Officials are investigating the fire as an arson.

6. February 29, Seattle Times – (Washington) State Patrol: Distracted driving caused
Highway 18 crash. One person was killed and a second individual was injured after a
vehicle crashed into a utility trailer that was pulled over on the side of road, prompting
the closure of a portion of Highway 18 in Auburn for approximately 2 hours February
29.

7. February 29, Lawrence Journal-World – (Kansas) K-10 reopens between 6th and
Bob Billings following closure due to accident. Kansas Highway 10 between Sixth
Street and Bob Billings Parkway was closed for approximately 2 hours February 29
while officials investigated the scene of a head-on collision.

8. February 29, WECT 6 Wilmington – (North Carolina) U.S. 17 reopens after log truck
overturns. U.S. 17 in Winnabow was closed for more than 4 hours February 29 after
the driver of a semi-truck lost control of the vehicle, ran into the median, and
overturned spilling logs onto southbound lanes.

9. February 29, U.S. Department of Labor – (Missouri) OSHA cites Sharpe Holdings in
death of worker at Missouri dairy farm. The Occupational Safety and Health
Administration cited Missouri-based Sharpe Holdings Inc., for 1 repeat and 17 serious
safety violations February 23 after a September 2015 fatal incident prompted an
investigation at the La Belle, Missouri dairy farm which revealed that the company did
not provide safety belts, secure passenger seats, or latch the rear doors of the vehicle
used on the dairy and cattle farm, and exposed workers to amputation hazards and
electrical hazards, among other hazards. Proposed penalties total $189,000.

10. February 29, U.S. Department of Labor – (Georgia) Worker’s severe saw injury
prompts OSHA investigation that uncovers more than a dozen safety hazards at
Augusta meat processing plant. The Occupational Safety and Health Administration
cited FPL Food LLC for 15 serious safety violations and proposed penalties totaling
$67,270 February 26 after an employee was cut by a hydraulic saw and hospitalized
December 2015, prompting an investigation at the Augusta, Georgia facility. The
investigation revealed that the company exposed workers to amputation hazards due to
unguarded cutting blades, fall hazards on platforms without rails, and electric shock,
among other hazards.

11. February 29, U.S. Food and Drug Administration – (National) Jack and the Green
Sprouts, Inc. is voluntarily recalling Alfalfa and Alfalfa Onion Sprouts. Minnesota
State health officials, Wisconsin State health officials, the U.S. Food and Drug
Administration, and the U.S. Centers for Disease Control and Prevention are
investigating an outbreak of food borne illness February 25 after routine disease
monitoring found seven E. coli illness cases had the same DNA fingerprint. Officials
are working to collect samples to determine the source of the outbreak and advised
consumers to avoid eating any alfalfa sprouts produced by Jack & The Green Sprouts.

12. February 29, Palm Beach Post – (Florida) Police: Ex-employee set car on fire, made
bomb threats in WPB, Riviera. A former sewage plant worker was arrested February
29 after setting his car on fire in the West Palm Beach sewage plant lot and claiming
that he had placed bombs at both the West Palm Beach plant and the Riviera Beach
Utility District, forcing an evacuation of the utility district. A device found at the West
Palm Beach plant was deemed a hoax and the site was declared safe.

13. February 29, WBTW 13 Florence – (South Carolina) 50,000 gallons of raw sewage
spills in Myrtle Beach, cleanup underway. Myrtle Beach city officials announced a
50,000-gallon sewage spill, near the Kampgrounds of America campground February
28 was caused by a failed air release valve on a sewer force main that leaked into a
nearby drainage ditch. Crews stopped the leak, disinfected the drainage ditch, and
worked to repair the air release valve February 29.

14. March 1, WLWT 5 Cincinnati – (Ohio) 4 hurt, 14-year-old student charged in
Madison Jr/Sr High School shooting. A student at Madison Junior-Senior High
School in Butler County, Ohio, was arrested and charged after entering the school and
opening fire during lunch in the cafeteria, leaving 4 students injured. The suspect ran
from the campus and dropped the weapon before being apprehended by police.

15. February 29, WJLA 7 Washington, D.C. – (Virginia) Bomb threat closes McLean
High School in Fairfax Co.; building later declared safe. Students were evacuated
and classes were dismissed at McLean High School in Fairfax County February 29
after the school received an emailed bomb threat. Police cleared the scene after nothing
suspicious was found.

16. February 29, WXIN 59 Indianapolis – (Indiana) IU confirms total of 8 mumps cases
on Bloomington campus. Indiana University officials reported February 29 that it will
provide another round of Measles, Mumps, and Rubella vaccines on campus March 3
following 8 confirmed cases of mumps on its Bloomington campus.

17. February 29, Glens Falls Post-Star – (New York) Water issue forces school closure
in Fort Ann. Low water pressure in the village prompted school officials to cancel
classes in the Fort Ann Central School District in New York February 29. The village
was placed under a boil water advisory for 48 hours after authorities found a leak in the
main line and determined that pumps at the water plant were not functioning properly.

18. March 1, SecurityWeek – (International) Qualys launches threat intelligence solution.
Qualys, a cloud security and compliance solutions provider, reported that its new
ThreatPROTECT solution will let clients visualize, prioritize, and take action to
minimize exposure from related vulnerabilities by collecting data via Qualys scanners
and Qualys Cloud Agents, which will compare the data with Real-Time Threat
Indicators (RTI) and provide clients with insights into which vulnerabilities should be
addressed first.

19. February 29, SecurityWeek – (International) Buggy ESET update blocks many
websites. ESET released version 13103 for its virus signature database after the
company received complaints that version 13102 prevented thousands of endpoint
users from accessing important Web sites such as eBay, Amazon, MSN, Google, and
several news sources, due to false detection of JS/ScrInject.B and HTML/Refresh.BC
trojan infections.

20. March 1, Meadville Tribune – (Pennsylvania) Former car dealer executive pleads
guilty to wire fraud. The former general manager of Community Chevrolet in Vernon
Township, Pennsylvania, pleaded guilty February 29 to Federal wire fraud charges after
he embezzled $485,800 from the car dealership from June 2008 – September 2013. The
former manager wrote approximately 34 company checks to a fraudulent marketing
company, Bullseye Marketing, gave a portion of the money to a co-conspirator,
deposited the majority of the money into personal bank accounts, and used the money
for personal expenses.

21. February 29, Salisbury Post – (North Carolina) Fire hits Burger King on Jake
Alexander Blvd. A Burger King restaurant in Salisbury, North Carolina, sustained
extensive damage February 29 due to a fire that began near the restaurant’s office and
storage area. Fire crews remained on site for several hours containing the blaze and
officials are investigating the cause of the fire.

22. February 29, Lansing State Journal – (Michigan) Fire damages housing complex in
Bath Twp. A February 29 fire at The Landings apartment complex in Bath Township,
Michigan, displaced dozens of residents and damaged 18 apartment units. No injuries
were reported and the cause of the fire was undetermined.
