VACANCY ANNOUNCEMENT
CLASSIFICATION TITLE: SOCIAL WORKER II
JOB LOCATION: ADULT SERVICES
Job Description:
Provide case management services to heavy care residents of Adult Care Homes so that
their needs and preferences can be met. This includes assessing the client's eligibility for
the service, reviewing the facility's care plans and provision of services, as well as
assessing the client's needs and developing and monitoring service plans. These activities
are done in partnership with residents, families, significant others, adult care homes, and
community service providers. This position also has responsibilities for adult services
intake on a rotating basis and other duties as assigned.
Minimum Training and Experience Requirements:
MSW degree from an accredited school of social work; OR, Master’s degree in a
counseling field and one year of social work or counseling experience; OR Bachelor’s
degree from an accredited school of social work and one year of social work or
counseling experience; OR Four year degree in a human service field or related
curriculum including at least 15 semester hours in courses related social work or
counseling and two years of social work or counseling experience; OR Graduation from a
four-year college or university and three years of experience in rehabilitation counseling,
pastoral counseling, or a related human service field providing experience in the
techniques of casework, group work or community organization.
COMPUTER PROFICIENCY IS REQUIRED. BI-LINGUAL ABILITY
(SPANISH/ENGLISH) IS DESIRABLE.

$33,258 annual

$27,258 annual

$25,938 annual
Position is full-time and benefited.
