Human Services Senior Practitioner Position Number – 2695BR Position is for an In Home Services Social Worker located at the Eastern Regional Center in Zebulon, NC. The worker will provide ongoing Child Protective Services to families that have been found to be Substantiated or In Need of Services. Worker will meet with the family within 7 days of case decision to explain and reiterate the need for ongoing services. Worker shall schedule and/or facilitate CFT Meetings to engage the family and support providers in developing the In Home Services Case Plan. The service plan shall be developed with the family to include the children and will reflect family’s strengths, address identified risk factors and the medical, mental health, educational, and well-being needs of each child. Worker will assist and work closely with parent/caregiver to achieve the goals of the service plan in order to reduce the likely hood of repeat maltreatment. Worker will conduct ongoing CFT Meetings in accordance with state standards and as needed. Worker will complete the Risk Re-Assessment and Family Strengths & Needs Assessments as required. The worker will maintain ongoing contact with parents/caregiver and the children in accordance with state standards and in a manner that ensures ongoing safety. Worker will also make collateral contacts twice per month with persons significant to the case. Worker will make ongoing diligent efforts to locate and engage absent parents throughout the life of the case. Worker will work closely with community partners to include; Law Enforcement, schools and Child Support. Worker may occasionally participate in court proceedings by completing court summaries and providing oral presentation. Worker may also complete CPS Assessments in accordance with state standards utilizing the NC Multiple Response System. Documentation must be kept in a thorough and accurate manner and according to policy. Worker will participate in a minimum of 24 hours of training annually and function as part of a Child Welfare blended team. *NOTE - A criminal background check will be conducted.
