Exam Policy and Procedure The Disability Services Office (DSO) at Boston College provides alternate testing facilities for students who have been approved for quiet space or extended time exam accommodations. DSO depends upon the integrity and efficiency of students receiving exam accommodations in order for all students to benefit from an alternate testing facility. The following information outlines the exam accommodation process. It is important to adhere to DSO’s exam accommodations in order for exams to be received, administered, and returned in a timely manner. Policy and Procedure Student provides appropriate documentation to DSO and registers with Dean Durrett to receive academic accommodations. Please refer to the DSO website for more information regarding registration instructions. Student meets with Dean Durrett to request and be approved for exam accommodations. Students are eligible to take exams in our office per their documented accommodations. Students must first discuss the scheduling of their exams with their professor when making an exam request. Should your professor raise any concerns about the integrity of your exam, they will make the final decisions regarding your exam eligibility and results. Exams are requested via our online form on the DSO. This can be found under the link named “Exam Accommodations Request.” Students submit the following information in this form 72 hours prior to the scheduled date and time of your exam: Name Your email address Course Number/Name Professor name Professor email address Date of exam Time of exam Amount of time given for exam Accommodation that you receive (i.e. time and a half, individual testing space, restroom breaks etc.) Date and time the class takes the exam Materials allowed in the exam (i.e. calculators, notes, etc.) Are you allowed to have drinks, snacks, or food during the exam? Professor contact information during the exam (i.e. phone number) Our office hours are 9AM-5PM, and all exams must be completed between those hours. Both student and professor will receive a confirmation email within 24 hours of submission request. During midterms and finals confirmation emails will be sent within 48 hours of submission requests. All students taking exams in the Disability Services Office must abide by the University honor code. Students who are not registered with our office will not be eligible to take an exam in our office, and students who register less than 72 hours prior to their exam will not be eligible until their next scheduled exam. Professors are requested to submit exam via email or in person to DSO. Student arrives in DSO at scheduled time to complete exam. DSO Exam Rules Student leaves all materials (i.e. backpack, phone) in DSO with assistant. DSO requires a completed exam accommodation form for each exam. Student will be instructed on what they may take into the testing room per the professor’s instructions (i.e. notes, calculator, scratch paper). A delay of 15 minutes after scheduled start time may forfeit testing period. Extended exam time may also be forfeited by arriving late. Student may not discuss exam during or upon completion of exam. If student is caught cheating, the professor will be notified immediately and will determine the appropriate disciplinary action, in accordance with university policy. In contract signing with Dean Durrett and by receiving exam accommodations, student agrees to follow the exam policies and procedures outlined by the Disability Services Office at Boston College.
