Private School Principals
NSLP B SNACK APPLICATION FOR 2007-2008
An Agreement Addendum, NSLP-Snack Site Application form, and a Monitoring Form are
enclosed for participation in the snack program. Instructions pertaining to each document are
listed below.
$
Agreement Addendum: An Agreement Addendum is required to participate and
is considered permanent. If new to the program, please complete this
addendum by indicating the name of the sponsor on page 1, and signing and
dating the second page. The completion and submission of the addendum will
update your continuous Agreement currently on file in our office, and allow the
claiming of after-school snacks. The Agreement Addendum sets fourth and
details the main regulatory requirements agreed upon by the sponsor.

NSLP-Snack Site Application: Snack Site Application to the NSLP-Snack
Program is required annually. The enclosed Snack Site Application form shall be
used when making application at the site level. The form allows for the
application of up to six (6) sites, and requires additional forms to be completed
for sponsors that have in excess of six (6) sites per year. Please indicate the
name of the sponsor, the date, an official signature, and complete the required
fields for each site. Also, note that the At-Risk determinations are based on the
most recent October percentage of needy figures.

Monitoring Form: This form was developed to assist you in your monitoring of
the NSLP-Snack Program. Sponsors are required to monitor the snack program
twice a yearBonce within the first four weeks of operation and the other time
during the school year. You are to keep these forms in your files.
$
Five-Day Snack Count or Daily Snack Count Form: They are provided to
assist with the Point-of-Service count. You are to keep these in your files,
also.
A complete and approved Agreement Addendum and the corresponding Snack Site Application
with original signatures must be on file in our office prior to the claiming of after-school
snacks. For your convenience, you may access the following documents online at

appears with upper and lower case letters). A CD with forms is available upon request. If you
