ADDENDUM 1 TO RFP 14-03E– Dedicated Internet Telecommunications Access Services
This addendum is issued as a part of the aforementioned RFP. The changes incorporated herein are an amendment to
and supersede those conditions shown in the original solicitation. The Proposer shall acknowledge receipt of this
addendum by signing this form and returning with their submittal. Failure to do so may subject Proposer to disqualification.
Q1: First, I wanted to check and make sure that the services you listed will not be in the midst of a contract on 7/1/2014,
at the beginning of the next E-Rate Funding Year.
A1: The anticipated contract start date for the awarded vendor for 14-03E is 7/1/2014.
Q2: Can you please confirm the service delivery address for the requested connectivity? If the street addresses for the
services are not listed on the Form 470, could you please send those to me as well?

Q3: Provide list all locations – school name, address, zip code and telephone number, incorporated in RFP – on an Excel
spreadsheet if available
A3: Our District utilizes a hub and spoke method to distribute internet access over our WAN. Currently all dedicated
internet access services are provided at our HUB located at 4204 Okeechobee Road, Ft. Pierce, FL 34947. Telephone
number is (772) 429-3600. This RFP does not seek WAN connectivity. So there is no need for respondents to evaluate
the school locations.
Q4: Bandwidth requirement per location/school
A4: Please see schedule 1 for various connection speeds requested. The location is 4204 Okeechobee Road, Fort
Pierce, FL 34947.
Q5: Would the district be willing to consider 5, 10 and 15 year options?

A5: Respondents must submit the 3 year rate as noted in schedule 1. You may also submit “additional” copies of
schedule 1 for other multi-year plans if you would like to. However, the selection committee will likely be evaluating price
comparisons for the 3 year plan to apply the selection points for that category to ensure fairness to all respondents.
Q6: There are no addresses listed on the RFP. Where does a vendor go to get addresses to conduct a site survey of the
buildings?
A6: The service address of our hub is located at 4204 Okeechobee Road, Fort Pierce, FL 34951. This is the only address
that connects directly to the internet. See Q3 for further clarification.
