Thank you for taking this survey, which seeks to gather information about your advising experience with
our College of Community & Public Service (CCPS). The programs in CCPS include:

This important feedback will be used to help improve the quality of advising and mentoring. Please
answer as many questions as you wish. Your mentor or advisor will not see this form; however the
students on the Dean’s Student Leadership Council and the faculty members in your program will be
informed of the summary of responses from all surveys.
CCPS has a dual advising system for undergraduates: an “academic advisor” on staff with the
Undergraduate Advising Center and a “faculty mentor” who is with the program’s department or school.
Graduate students have a single “faculty advisor” who is with the program’s school, although the
Graduate Program Director, if different, may also do substantial advising. Sometimes faculty and staff
members who are not officially assigned will serve as a student’s advisor. This survey will use the term
“advisor/mentor” to apply to all these people.
