MMR (Measles/Mumps/Rubella) vaccine. Required before entering Pacific University
and the School of Physical Therapy. Series of two.
Immunization Required before attending Clinical rotation:

Td/Tdap – Tetanus-Diphtheria-Pertussis.
TB (tuberculosis) skin test.
Hepatitis B virus (HBV) vaccine. Series of three and Titer.
(Given in three doses; the second dose is given one month after the initial injection and
the third is given six months after the first.)
Varicella (Chickenpox) vaccine. Series of two.
Hepatitis A virus (HAV) vaccine. Series of two.
Note: If it is unknown if you have had any of the vaccine’s above, Titer tests can be taken to
determine if you need to proceed with the vaccine.
