APPENDIX 6 TO ANNEX F TO CFC OPLAN (KOREA) 9518X-XX
MEDIA REGISTRATION
1. News media representatives (NMR) are accredited at the Host Country Ministry of Information upon
arrival, and then registered at each major subordinate command. CFC PAOs will register all NMRs and
issue an appropriate badge for that MSC. NMRs without the correct accreditation/registration badge will
be escorted to the nearest public affairs office or commander. To enforce this requirement soldiers will
not talk to media representatives who cannot display the correct accreditation/registration badge. This is
not a punitive measure, merely an attempt to identify numbers of media in each respective area.
2. To be accredited/registered, and to maintain their accreditation/registration, NMRs must agree to follow
the established media ground rules, and provide the following documents and information:
a. News media representatives desiring to cover CFC forces must satisfy the following
requirements for registration:
(1) Provide picture ID and Host Country Ministry of Information accreditation badge.
(2) Have proper equipment and clothing.
(3) Agree to abide by general and specific ground rules. Appendix 3.
(4) Have necessary personal equipment and supplies to support themselves.
(5) Fill out and sign Tabs A through F to this Appendix.
3. Once paragraph 2 is complete, the JIB will fill out a press badge, photograph the NMR and laminate
the photo onto the badge.
4. NMRs who operate in subordinate AOs do not have to resign media ground rules agreements.
Possession of a press badge certifies that accreditation/registration agreements have been completed.
Subordinate PAOs will remind NMRs that all agreements are still in effect.
5. All media on the accredited list will have read and signed the ground rules.

AIR TRANSPORTATION RELEASE/HOLD HARMLESS AGREEMENT
I
(Full Name), am about to take a
flight or flights as a passenger in aircraft operated by or on behalf of the United States of America,
including such other transportation by this or other means that may be reasonably required, on
(Date or Dates).
I understand that in transporting me, the United States Government is not acting as a common
carrier for hire and does not bear the liabilities attaching to that status.
I acknowledge that I am doing so entirely upon my own initiative, risk, and responsibility and that I
am under no compulsion to do so.
In consideration of the permission extended to me by the United States, through its officers, to
take this or these flights, I do, for myself, my heirs, executors, and administrators release and forever
discharge, and agree to hold harmless the Government of the United States and all its officers, agents,
servants, subdivisions and employees, acting within the scope of their employment, from any and all
claims, demands, actions, or causes of action which may arise in my favor, for property damage and/or
personal injury or death resulting from said flight(s) or continuances, resulting from any negligence or
otherwise on the part of the Government or its officers, agents, servants, subdivisions, and employees, in
connection with the above flight or flights, as well as any ground and flight operation incident to the actual
flight or flights.

AGREEMENT TO REIMBURSE
1. I understand that covering combat and other military operations may require that I be provided certain
items not locally available or convenient due to combat operations.
2. These items may include, but are not limited to, meals and air travel into and within the theater of
operations.
3. I agree to reimburse the US Government for any items it may provide me while I am in the CFC area of
operations.
4. I understand that my agreement to this statement is a condition of being registered to cover US military
operations and receiving assistance for that coverage.

HOLD HARMLESS/RELEASE FROM LIABILITY STATEMENT
1. I recognize that covering combat and other military operations carries with it certain inherent risks to
life, limb, and equipment.
2. I acknowledge that the US military, in pursuing the accomplishment of mission, cannot guarantee my
personal safety or the safety of my equipment.
3. I hereby release the US Government and the US military of any liability from, and hold them harmless
for, any injuries I may suffer, or any equipment that may be damaged, as a result of my covering combat
or other military operations.
4. I understand that my agreement to this statement is a condition of being registered to cover US military
operations and receiving assistance for that coverage.
