THOMAS LAKE SAFETY PATROL TEAM

Dear Parents/Guardians,
The Suburban Law Enforcement Association (SLEA) will be sponsoring the School Patrol Appreciation
Day at Nickelodeon Universe in the Mall of America, on Tuesday, May 6th, 2014, from 10:00 a.m. - 2:30
pm.
Again this year, the SLEA will provide each School Patrol Member with a special wristband at a
discounted price of $5.00. This wristband will allow the patrols UNLIMITED free rides from 10:00 a.m.
until 2:30 p.m.
The cost for transportation to the Mall of America will be $6.00 per patrol.
Lunch will be the responsibility of each person. We recommend students bring a sack lunch that
can be placed in a backpack or tote of the chaperone.
If your child wishes to purchase any refreshments, souvenirs, or arcade games they will need to provide
their own money.
In order for your child to attend this special event, please fill out the bottom portion of this permission
slip and have them return it along with $11.00 (in cash or check made out to Thomas Lake) to Mr.
Groenewold by Tuesday, April 8, 2014.
We are in need of approximately 15 volunteers to help chaperone. Please leave your name and phone
