Dear student,
The End examination of Diploma in Opthalmic Technical Assistant (DOTA) Programme will be
held in May 2015. Time table declared on YCMOU Portal.
01. The students have to pay examination fees Rs. 350/- per theory paper and Rs. 100/- for Marksheet and
Postage Fee by Bank Challan (Central Bank, Bank of Maharashtra & Bank of India only).
02. This Examination Form is for “Repeater Student only’’. Fresh student of Diploma in Opthalmic Technical
Assistant (DOTA) (New Syllabus) need not fill this Examination form.
03. Student should attach Xerox copy of last examinations mark sheet and challan along with the Examination
form.
04. Student should record correctly (tick marks 3) the subject for which he/she wants to appear.

After last date of submission of examination form to Regional Office, Study Centres paid Rs. 200/- per day.
06. The examination centre will be alloted by the university and you will get hall tickets at your study centre or
you can download it from university website http://ycmou.digitaluniversity.ac.
07. You should be in touch of your examination centre for seating arrangement one day in advance. You
should be present at least half an hour before the examination time.
08. Result will be declared on university's website http://ycmou.digitaluniversity.ac. After declaration of
result, you can apply for “verification of marks” within 15 days. Online form should fillup on university
website. The fee for Marks Verification is Rs. 100/- for each Course.
09. The student must bring his/her hall ticket and Identity Card/Identity Proof and produce the same when
demanded during the examination sessions.
10. The student should not keep any books, papers or other objectionable material for copying, during the
examination. If student is found, copying during the examination or during the Central Assessment Program
a Legal action will be taken on him/her as per the provisions of 1998 Maharashtra Act no. 31.
11. All the journals should be submitted to the Study Centre Head upto 10th April 2012.
