Macmillan Cancer Support Quality Environment Mark awardees Template for service profiles on NHS Choices Macmillan Quality Environment Mark (MQEM) team and NHS Choices have provided the following proposed template for the kind of information you should consider adding to your service profile on NHS Choices. You may also wish to see existing service profile examples to help shape what you publish on your profile. • Name of service Including any variations that people might have to look for on hospital signage • Description of service Provide an overview of the service the awarded environment houses. This may include information such as: The ward: Describe the configuration of the ward, whether there are private rooms, how many beds are available. For example: We have a 6 bedded palliative care ward; or We have a 10 bedded oncology ward The day unit: Describe the configuration of the day unit. For example: We have a 8 chaired day oncology unit and a radiotherapy unit. The Information and Support centre: Describe the services offered though the information and support centre. For example: whether there are rooms for support groups, reading rooms, complementary therapy rooms • Things you should know about us Where the service is located/how to access the service Provide details of where the service is located and the access to the service including lifts, stairs, routes designed for accessibility, signage in the hospital. Opening hours or visiting hours Provide information on the opening hours of the service, or the visiting hours and restrictions Parking The overall location profile will have details of car parking and a map to the service location. You may wish to provide further information around car parks which are closest to the service, blue badge parking near your service, charges associated with parking to access the services. What patients and visitors can bring to the service You may wish to provide information on what patients can bring if using the service. For example: own pillows and bedding, comfortable cushion if attending a support group. • Staff profiles Our staff You can provide details from the kind of staff that work for the services, how they work together as a team and individual staff profiles with pictures. • MQEM status You can access a jpeg of the MQEM logo to add to your profile page here You may wish to include details on what the MQEM award means to the service, when you were accredited, next review date etc • Patient feedback Patients are invited to provide comments and feedback on services from each service profile. We recommend you encourage your patients to use this feedback facility to capture comments and feedback to help support your reapplication for MQEM. Managing feedback through responding to comments can be done as a comment administrator.
