This form is to be completed by the departmental Graduate Program Chair for each nominee for the MYF Funding
Program and must be accompanied by a copy of the DGS admission offer letter.

Scholarships will be disbursed in three (3) equal installments per year unless an alternate payment schedule is requested by the

D) REQUESTS
Eg. Only disburse scholarship payments in Fall and Summer, the student will TA in Spring
Fifth year of funding (consisting of a tuition award and departmental contribution). This is optional and only for units willing to
contribute $18,000 in the fifth year. Please tick if you wish to nominate the student for a fifth year of funding.
