CHAPTER 7440 – EFFLUENTS
7441.23 – Reporting
Establish a 7440 case folder for each waterborne wastewater disposal system. Maintain the case
folder at the District Office files with an optional copy in the Supervisor’s Office files. As a
minimum the case folder shall contain the following:
1. Engineering reports and related correspondence.
2. Contract package (drawings and specifications).
3. Detailed construction “as-built” or detailed sketches of the entire wastewater system
(see FSM 7413.6).
4. Detailed operation and maintenance (O&M) plan for the entire wastewater system (see
FSM 7413.9 and FSH 7409.11 sec. 71).
5. Copies of all past condition survey reports, and records of what was done on system as
a result of each report (see FSM 7413.8 and FSH 7409.11 sec. 71.3).
6. A record of all action (maintenance, replacement, additions, pumping) performed on
the wastewater system.
7. Copies of all correspondence related to the system.
