For all projects using herbicides within the scope of the Final Environmental Impact Statement (FEIS) "Managing Competing and Unwanted Vegetation," report any worker health effects (injuries or exposures) to the Regional Director, Forest Pest Management (FPM). FPM shall maintain these reports in the Federal Records System for a period of 30 years, and issue a statistical summary of these reports annually. Use file designation 2150-5. Group the reports by fiscal year, under the title "Vegetation Management Human Health Effects." Place a notice on each record in the files that the record contains data which may be exempt from disclosure under FOIA Exemption 6. Do not release health effects information which identifies an individual to any person other than the injured party. 53 - WRITTEN REPORTS. Submit copies of all human health effects forms (R6-FS-2100-10/10S; R6-FS-210012; CA-1; CA-2) resulting from a project to manage competing and unwanted vegetation to the Regional Director, Forest Pest Management.
