In addition to the notification requirements in Exhibit 1, for projects within the scope of the Final Environmental Impact Statement (FEIS) "Managing Competing and Unwanted Vegetation," notify the Regional Director, Forest Pest Management (FPM) of all injuries, illnesses, or herbicide exposures which occur as a result of the project. Submit the forms specified in Section 33.1 to accomplish notification. In addition to the notification requirements in Exhibit 5, for all projects using herbicides within the scope of the FEIS "Managing Competing and Unwanted Vegetation," notify the Regional Director, Forest Pest Management (FPM) and the Regional Integrated Pest Management Specialist of all injuries, illnesses, or exposures. Submit the forms specified in Section 33.1 to accomplish notification. 33 - ACCIDENT REPORTING. 33.1 - Types of Reports. For projects within the scope of the Final Environmental Impact Statement (FEIS) "Managing Competing and Unwanted Vegetation," submit forms R6-FS-210010/10S, R6-FS-2100-12, and copies of CA-1, and CA-2 to the Regional Director, Forest Pest Management (FPM) as required by the "Guide to Conducting Vegetation Management Projects in the Pacific Northwest Region." 33.3 - Form FS-6700-8, Report of Incident to Other Than Employees. For projects within the scope of the FEIS "Managing Competing and Unwanted Vegetation," in addition to any other forms required by this handbook, submit Form R6-FS-2100-10/10S to the Regional Director, Forest Pest Management (FPM) to report human health effects to those other than employees. 37 - REVIEW AND ANALYSIS. Forest Pest Management (FPM) shall maintain all reports of human health effects received from projects within the scope of the Final Environmental Impact Statement (FEIS) "Managing Competing and Unwanted Vegetation" in the Federal Records System for a period of 30 years and, through the Regional Forester, shall issue an annual statistical summary of these reports. Do not release health effects information which identifies an individual to any person other than the injured party.
