Use form R2-6600-1 to document request for new IBM profiles. Instructions for completing the form are on back of the form. Submit completed form to Information Resources Management staff in the Regional Office or the system administrator at the Supervisor's Office for approval by the Employee's Supervisor, Project Leader, or appropriate management official. Manage the security of ID's and passwords according to DM-3140-1, Appendix D. Employees must immediately change password assigned by system manager at the time profile is established. Do not share individually assigned passwords. Unauthorized sharing of passwords may result in appropriate disciplinary action. 1. Passwords must be eight characters or greater and be a combination of alpha, numeric, and special characters. Do not use personal data such as birthday, pet's name, children's names, and so forth in password. 2. Change passwords for interactive applications at least every 90 days and passwords for batch applications at lease every 180 days. 3. Change passwords immediately if it is suspected the password may have been compromised and report the incident to supervisor and system security officer.
