ASSOCIATE OF APPLIED SCIENCE PRELIMINARY REQUIREMENTS CHECKLIST
MINIMUM REQUIREMENTS








1. Complete the Clark College Application for Admission online at www.clark.edu. Students who are new to
Clark College must also pay a one-time non-refundable $25.00 application fee; all fees subject to change.
2. Submit official college transcripts from ALL college attended and send updated official transcripts each term.
3. Complete the following preliminary requirements with a minimum grade of "C" (2.0) or higher:
English Composition I
Anatomy and Physiology I, II, III
Intermediate Algebra
• ENGL& 101……3-5 credits (Must be 7 years current upon program entry) (Must be 7 years current upon program entry)
Medical Terminology I & II
• BIOL& 251……4 credits
• MATH 093 or 095…….5 credits
• BMED 110……..3 credits
Intro to Medical Radiography

Proof of College Level Reading with a COMPASS Reading Test Score of 74 or higher

7. To apply to the program, complete the Clark College Application for Admission and the Medical Radiography program
Statement of Intent forms. The Statement of Intent requires a $50 Health Occupations application fee (all fees subject
to change). Return completed materials to the Clark College Enrollment Services department.
8. To qualify as a Lower Columbia College (LCC) applicant, students must reside in the LCC Service District, and complete
a minimum of 15 credits of applicable preliminary coursework at LCC. Students must indicate on the Statement of
Intent the selection pool in which they wish to be considered, and understand that an applicant can only be considered in
one selection pool. Four students selected will be from the LCC Service District. Applicants are competitively ranked
within each applicant pool.

UPON ACCEPTANCE
Prior to entrance into the first quarter of the program students must complete the following

Upon acceptance, all students must submit a non-refundable $200 deposit to reserve his/her position in the program.
First Aid & Health Care Provider CPR (may substitute with current American

Heart Association Healthcare Provider level CPR card and complete a First Aid course)
AIDS Education—Students can earn this certificate by completing the course OR by
HEOC 120

watching the 7-hour AIDS Education tapes. To reserve the tapes call (360) 992-2939.
Immunizations & Physical Exam (See Upon Acceptance section on the MRAD website)
Comprehensive Criminal Background Check required (See "Criminal Background Check" on website)
FACPR 032
Students may be subject to a drug screening at the discretion of the clinical site. Drug screening costs are the
responsibility of the student (up to $48). If the clinical site finds the student’s drug screen to be unsatisfactory, the
student may be dismissed from the program (see substance abuse policy).
A minimum of 16 hours of volunteer or paid experience in the field of Medical Imaging, 8 hours must be in hospital
setting (letter from employer/volunteer coordinator required prior to starting the program)
