Instructions for registration:
1. Print this packet. It will come with the official team entry form, player eligibility form,
estimated arrival form, Code of Conduct agreement and 16 player waivers.
a. Team entry form – must be fully completed and signed off on by IMS Professional before
it will be accepted
b. Player eligibility form – all players must put legal name, school ID and registrar for your
school must verify eligibility of class hours and stamp/sign form. Again, IMS
professional must sign off
c. Estimated arrival form – let us know when you are coming in. It will help us with
scheduling. If you live within four (4) hours of Nacogdoches, plan on playing one game
on Friday, 11/4.
d. Player Code of Conduct – all players must read, print and sign their names. There are 16
slots on the form for all players to sign on one form.
e. Waivers – there are 16 waivers attached to this entry packet. Each individual player must
complete their own individual waiver. Players can complete them on site when they
arrive, but it will be easier if they get completed and sent in with all other paperwork.
2. A team may submit a blank team entry form and payment to hold a spot for the tournament.
3. All payment by credit cards will be taken over the phone with Janice Casper. When you
paperwork is received, we will contact the person listed on the team entry form to arrange
payment. Receipts can be mailed, scanned and emailed or held for pick up on site.
4. Roster changes – will only be approved up until registration is done on Friday night in the
Student Recreation Center. A player must have all forms of documentation on hand to be
added to a roster (new roster form, new eligibility form, new code of conduct and new
waiver). All documents can be individually printed from tournament website. Failure to have
these papers will result in your player not be allowed to play.
5. ALL PLAYERS MUST HAVE THEIR STUDENT ID ON THEM AT ALL TIMES.
Drivers licenses may be acceptable if you are on the roster. No photo ID, no play.

Tournament play starts as early as 2:00p-3:00p due to team
numbers and our desire to finish at an appropriate time on
Sunday so as teams can travel as safely as possible back home.
Please give us some idea of your travel plans. If you live within
a four hour radius of Nacogdoches (roughly DFW, Houston,
Austin, Sulphur Springs), plan on playing one game Friday,
November 5.
