What We Have to Offer
• Our Health Clinic and Personal Counseling are both located in 104 West Quad in the student dormitory quadrant.
• Our Health Clinic hours are 10 a.m.-4 p.m.
• Personal Counseling hours are 8:30 a.m.-5:00 p.m.
• Our Personal Counseling staff includes:

• Marjorie Lang, MA, LLPC
• Natalie Wicks, MA, ACSW
• Our Health Clinic is staffed by a Nurse Practitioner.
Individual sessions are offered in a confidential environment using an empathetic approach. We provide counseling to students for matters of concern including:
• Adjustment issues
• Relationship difficulties
• Stress
• Anxiety
• Depression
• Self-confidence/esteem issues
• Addiction
• Grief and loss
• Trauma
• Psychological distress
• Our services are provided on a walk-in basis and by scheduled appointment.
• Students utilize our services for a variety of reasons including:
• Health maintenance and preventive services
• Illness and injury care
• Annual health exams
• Annual sports physicals
• Required health physicals for medical and nursing students
• Sexual health (male and female)
• STD screening and education
• Health education and promotion
• Diet, nutrition, and weight counseling
• Immunizations including flu shots
• TB skin testing
• Referrals
• Personal Counseling visits are included within tuition.
• Health Center visits are provided in cooperation with Collegiate Insurance Resources (CIR).
• A student health insurance plan is required for all students living in the dorms: student insurance
(CIR) or private insurance.
• All students have access to health clinic services.
• Students living in the dorms on campus are required to maintain health insurance at all times throughout the academic year.
• Cofinity provides health insurance for all students. The cost is provided below:
• $1,391 for undergraduate students per annum, and,
• $1516 for graduate students per annum.
• Students may opt out of Cofinity-provided health insurance by completing required online opt-out documentation by September 26,
2015.
• A link to Cofinity is provided on our UDM Wellness Center website.
• The procedure for opting out of student health insurance is detailed in a one-page document included in your SOAR parent folder.
• Failing to complete and submit the online Cofinity Health Insurance waiver form will result in the following:
• A charge for the full premium and corresponding hold on the student
account;
• Automatic enrollment in Cofinity Health Insurance for one year.
• Only dorm residents need to waive student health insurance. This procedure and process does not apply to commuter students.
• The UDM Wellness Center requires that a Tuberculosis skin test report (TB
Questionnaire) be on file for all new international students and new incoming freshman students.
• A TB skin test or chest x-ray must be performed in the U.S. or Canada within 12 months of the student’s first semester on campus.
• Failure to complete and provide the TB Questionnaire, and to obtain screening, as needed, will result in a hold being placed on your student’s account and an inability to register for the next semester.
• Please make sure that your student signs and submits the TB
Questionnaire by mailing it or dropping it off at the Wellness Center.
