A Detailed How-To For Providers

ePDP includes the
elements of a PDP
as well as a full templated
discharge summary.
Start this document early
during the admission to
ensure smooth discharges
from the hospital.

The Summary tab
displays all
information that has
been entered so far.

searching from the
drop-down menu.
Remember you can
save anytime by hitting
Save at the bottom of
each section.

hospital course
Enter all other diagnoses
with associated hospital

If PCP within DPH/UCSF system, an email will be generated advising PCP that
dc documents available for review. Additional physicians can be selected in the
Additional Distributions. Alternatively free text the name of the physician if not
in the DPH/UCSF system. No email will be generated to these providers.

Summary.

ePDP by clicking on the field
and then hitting modify below
or go directly to the relevant tab
above.

To FINALIZE the
ePDP go to the
Summary tab and
click Finalize below.

IMPORTANT: Your signature
on the FINAL ePDP indicates
that you are accountable for the
entire content (even if multiple
providers contributed to the
document over time).

Med Rec and Discharge
Instructions must also be
completed for the patient to
be discharged.

To make edits to an already
finalized ePDP, go to
Enter/Update PDP. Click
Continue to make changes to
the document.
