We are searching for a Case Manager to join our Supportive Housing for Familiesteam: Supportive housing for Families is a child welfare (DCF) family reunification/preservation program, which provides intensive case management to qualifying child welfare families. The Case Manager will provide assessment, intensive home-based case management and housing assistance for families to ensure the safety and well-being of their children. The Case Manager will respond effectively to meet the needs of clients and their children, staff, community members and funding agents. Maintain up- to-date and accurate manual and electronic records. Provide transportation for families as needed. This position is 35 hours/week, Monday – Friday. Nights and weekends as needed. Position is located in Bridgeport, CT. Requirements: Bachelors degree in social work or other related human services field is required Two years of experience in a similar position Strong written and verbal communication skills. We are searching for a Registered Nurse (RN) Supervisor to join our Home Care team: The mission of FCA’s Home Care program is to prevent premature institutionalization, promote the highest quality of life and strengthen families and individuals in life’s transition and crisis. The Registered Nurse Supervisor must ensure quality and safe delivery of client services and ensure compliance with regulations, standards, policies and procedures. Essential responsibilities include but are not limited to the below: Coordinates and manages all services rendered to clients, client families by direct service staff under her/his supervision. Visit and complete assessments of clients receiving home health aide services as often as necessary based on patients’ condition but not less than every thirty days. Refer patients to appropriate sources when client’s condition and care needs require more than supportive services. ( e.g. skilled services) Supervising assigned personnel in the delivery of services to our clients and families including the home health aide, homemaker, companion, chore, personal care assistants, escort and carpenter. Requirements: A baccalaureate degree in nursing and at least two (2) years full-time clinical experience within the past five (5) years in a home health care agency or related health care facility/program which included care of the sick; or An associate degree in nursing and at least three (3) years of full-time clinical experience in nursing within the past five (5) years, at least two (2) of which were in a home care agency or related health care facility/program which included care of the sick; or A diploma in nursing and at least three (3) years of full-time clinical experience in nursing within the past five (5) years, at least two(2) of which were in a home health care agency or related health care facility/program which included care of the sick.
