Please answer the following questions: 1. *Commitment The Construction Contractors’ College program is a seven-month commitment. Do you commit to completing the entire seven-month program with all of the requirements? YES or NO 2. *Business Duration To qualify for this program you must have an active established business that has been in existence for a minimum of one year. YES or NO If Yes, how long? 3. *To qualify for this program, your business must have filed appropriate tax returns with the IRS for at least one year. *Have you filed 1065, 1120, 2210 or Schedule C with the IRS? You are required to submit a copy of your most recently filed tax return (tax year 2014 or tax year 2015) with the application. 5. *Business Construction Specialty To qualify for this program your business must specialize in the area of construction or construction services. Does your business specialize in construction or construction services? YES or NO If you provide construction or construction services, what type do you provide? 6. *Organization Type/Business Formation Are you a Corporation, Sub S Corporation, Partnership, Limited Liability Company or Sole Proprietorship? If Yes, list the organization type/business formation. Note: If a partnership, partners must authorize approval to participate by uploading a simple statement below and note the percentages of each partner. You are required to submit a copy of your supporting documentation of the organization type/business formation along with the application. 7. *Majority Owner Are you the majority owner?
