The field note (OM1-31) will be updated and Appendix A added as described below. The LOINC encoded AOE examples in Appendix Aare provided as guidance and focus on commonly used Ask at Order Entry questions. Miscellaneous edits to AOEs without LOINC codes to prepare for submission to Regenstrief to obtain LOINC codes. 10 Page 3 of 22 11 12 13 14 15 16 Electronic Directory of Service (eDOS) Implementation Guide Ask at order Entry (AOE) Questions Appendix A – Ask at Order Entry (AOE) Question Examples The initial text below is excerpted from HL7 Version 2.5.1 Implementation Guide: S&I Framework Laboratory Orders from EHR, Release 1 – US Realm, Section 2.6.5 Ask at Order Entry (AOE) Observations, to explain how Ask at Order Entry questions are “answered” in a laboratory order. 17 18 [Begin excerpt] Ask at Order Entry (AOE) responses are recorded as observations that provide critical information for the calculation or interpretation of some lab results or to satisfy state and federal health agency mandated information gathering requirements, e.g., for blood lead testing. Not every order will have the need for AOE questions and associated observations. Examples of the type of information gathered from a patient include employment information, pregnancy status, the date of the last menstrual period, mother’s age, and questions about family and personal history. AOE responses can take several formats, including but not limited to: Yes/No (and coded) to answer questions like “Is this your first pregnancy?” A code drawn from a value set to provide a coded response to, e.g., “What ethnicity do you consider yourself to be?” A number with units for the mother’s age A date format for the patient’s last menstrual period. In some cases there may be AOEs that request the outcome of previous results phrased as a question, for example “Was your previous pap abnormal?” Should the need arise to send the actual previous result the “previous result group” in the message structure should be used. The lab will indicate if and which AOEs to include with the order in their test compendium, either using paper or electronically via the “Implementation Guide: ACLA Test Compendium Framework (eDOS), Release 1, January 11, 2011”. In eDOS, there is a repeating field, OM1-31 (Observations Required to Interpret this Observation), that lists all observations that must be captured at the time the order is created. Each instance in the OM1-31 (Observations Required to Interpret this Observation) represents a single AOE question. The eDOS also indicates how the OM1-31 (Observations Required to Interpret this Observation) shall be conveyed in the order message – currently as AOE responses in OBX segments or, in the future, other appropriate HL7 message fields elsewhere in the message. The decision regarding where to message AOE information is outside the scope of the LOI guide. LOINC shall be used as the standard coding system for AOE questions when an OBX segment is used and if an appropriate LOINC code exists. The LOINC and local code describing the question will be placed in OBX-3 (Observation Identifier). Appropriate status is defined in the LOINC Manual Section 11.2 Classification of LOINC Term Status. OM1-31 lists all observations that must be captured at the time the order is created. Multiple AOE questions are allowed for an individual orderable test; each instance in the OM1-31 represents a single AOE question. 36 37 38 39 40 Because Meaningful Use has supported “Structured Data” for lab results, beginning with Meaningful Use Stage 1, the AOE examples below suggest HL7 data types to be used when the EHR system returns the “answer” to the lab’s compendium test question. For example, the XAD (Extended Address), data type is suggested when reporting the employer address “answer” for blood lead test question about employer address. 41 42 43 44 The LOINC encoded AOE examples in the table below are provided as guidance and focus on the most commonly used Ask at Order Entry questions. This is not an exhaustive list and does not attempt to include all possible AOEs. Implementers can incorporate additional AOEs as needed for their specific environment. 45 46 47 48 49 LOINC codes may change in newer versions of the data base so it is suggested you verify that the LOINC codes are valid before using. Some LOINC codes in the list below were “Trial” codes at the time of publication and are designated with an ‘*’ following the LOINC code. Also I did find an animal rabies specific value set: PHVS_AnimalSpecies_AnimalRabies 2.16.840.1.114222.4.11.3068 in PHIN VADS I don't have any idea, who is filling these answers out - I think it is probably a mix - if the sample is from an animal, then I would think it should be the vet? Rabies in saliva of bats for 24 days, although unknown for all species this is longest I found. We encourage implementers to use the HL7 defined message structure where possible. Should we not a comment to PID 07 like we had in LRI that age at time of collection should be transmitted as an OBX sub SPM for specimens collected at time of order? Not persuasive with mod 21-Feb-2013 - Discussion on whether AOE OBXs should be under PID, ORC/OBR, and SPM, or always under ORC/OBR is being discussed in OO in general. Current DoB/Age is always on PID, while other ages would AOE. Race and Ethnicity can vary, so the expression of the patient may be different than what is clinically required to interpret the test. They will therefore be AOEs. Gender is always on PID. Motion to clarify that AOE is always sent at ORC/OBR level, while other information gathered during ordering and specimen collection is communicated on either PID, ORC/OBR, or SPM, whatever is appropriate. Riki Merrick, Ken McCaslin 86 87 Ask at Order Entry as OBX Should we send AOE ONLY in OBX – some answers would already be available in other fields in the message like PID-10 – was discussed with Ted Klein and in order to communicate where else would be pretty difficult, so more simple to always use OBX, even if it will create duplication ORC/OBR pair uses OBXes to convey this, although OBX under PID or SPM could be considered as well. Either one of these methods would make it clear they go with the order and are not an actual result. Page 18 of 22 Electronic Directory of Service (eDOS) Implementation Guide Ask at order Entry (AOE) Questions Need to define how to echo the AOE back that were used for the test interpretation in the result how those would be differentiated from the results – create a flagging (use existing field with specific value, or add a new field) in the OBX or a separate grouping In ELR we created a separate OBR heading for these – other guides suggest to use them under the SPM or the PID (answer applies to all orders) Most consistent do send them under the ORC/OBR that they were ordered under.
