include the pledge term (i.e. the latest a student can join is spring term, junior year).
o 3.0 cumulative GPA* If below 3.0 please answer question 4 described on page 2
o Motivated and willing to dedicate 3-4 hours per week to fraternal activities.
o Must be available 6-9 pm on April 22nd, May 6th & 20th No exceptions!

take place on Friday, Saturday or Sunday, April 11th, 12th or 13th. Please circle all time slots in
which you are available for an interview. Interviews are approximately 20 minutes long. If you
are available for part of the hour, please note which part accordingly.

Grade Release Authorization:
I give my permission for the Alpha Kappa Psi Vice President of Membership at the

determining my eligibility for membership in the Kappa Chapter. Once a member, my
records may be checked by the Vice President of Membership for the purpose of determining
chapter GPA.
