Are you interested in participating in a research study? We are looking for volunteers to participate in a research study examining the effectiveness of our GMAT preparation course and its usefulness in our MBA admission process at TAMU-SA.

Participants will receive a $100 discount.

 Must register for GMAT prep class (less $100 discount)

 Must complete an informed consent form for research participation (located on the back of the course registration form)

 Must print/save and submit to Dr. Hurley a copy of the results of your first GMAT practice test from the September

14 th practice test date.

 Must authorize the release of OFFICIAL GMAT test score submitted to TAMU-SA for admission (GMAT school code

7B7-9D-05)

 Must complete an attitude survey and a post-class evaluation form

 Must authorize a release of undergraduate courses, major, and GPA information

 Must authorize a release of grades from MBA coursework.

 All information is confidential and procedures will follow the privacy guidelines established by university policy.

GMAT Effectiveness Study

AGREEMENT TO PARTICIPATE

I, , agree to participate in a study involving the effectiveness of a GMAT preparation class and test at TAMU-San Antonio.

I understand that I will be expected to complete a number of forms, checklists, and questionnaires relating to my knowledge, attitudes, and behavior. These instruments may include behavioral or attitudinal surveys, demographic and work experience information, GMAT practice and GMAT official tests. In addition, I understand that I am required to participate in and attend a 10-session GMAT preparation course (schedule attached), authorize the release of GMAT practice and official test results, undergraduate major and GPA records, as well as graduate GPA and grades from my MBA program.

I have been informed that any information obtained in this study will be coded so as to protect my privacy and confidentiality and upon collection of all data, all personal identifying information will be destroyed. I further understand that my name will not appear in publication or print nor in anyway will my name be publicly associated with my GMAT test results, GPA or any other study-related information. Under this condition, I agree that any information obtained from this research may be used in any way thought best for publication or education.

I understand that there is no personal risk or discomfort directly involved with this research and that I am free to withdraw my consent and discontinue participation in this study at any time. If I have any questions or problems that arise in connection with my participation in this study, I should contact the project director (Dr. Tracy
