Fall 2011 Master’s Pre-Entry Checklist Welcome to the School of Nursing! This checklist will guide you through the steps you need to take to ensure a successful start at Sacramento State. Intent to Enroll (ITE). If you have not already done so, please accept or reject your admission to the University immediately at www.csus.edu/admissions/ite. If you do not complete your ITE, your admission may be rescinded and you will need to reapply for a subsequent term. We don’t want to lose you! Conditional Admission. If you were admitted “conditionally,” you must complete the conditions outlined in your admission letter to continue in the master’s program. Possible conditions include GRE score, completion of bachelor’s degree, or RN licensure. If these conditions are not met by the deadlines provided, you will be disenrolled from the master’s program and you will need to reapply for a subsequent term. Again, we do not want to lose you! Once you have satisfied the conditions, please submit proof of completion and a Classification Application form to the School of Nursing drop box in the nursing lobby or by U.S. mail (attention Teri Tallman), http://www.csus.edu/gradstudies/forms/candidacy_formspetitions/classification_application.pdf. Please note our physical and mailing addresses are different! We will obtain faculty signatures, put a copy in your student file, and submit to the Office of Graduate Studies to change your admission status from “conditional” to “classified.” SacLink Account. If you have not already done so, please go to www.saclink.csus.edu to set up your SacLink account. This will allow you to access My Sac State. My Sac State is your personal portal to important campus messages, your personalized “To Do List,” payment and account information, where you will register for classes—and more! Final Transcripts. If you have not already furnished the Office of Graduate Studies with final official transcripts documenting all college course work and dates of any degrees granted, please do so immediately. You will not be eligible to enroll in any subsequent semester if your records are incomplete, and/or your admission may be rescinded. We recommend you enroll in NURS 209 and NURS 210. If you only want to take one course, we recommend NURS 209. If you plan on taking more than two courses, you MUST discuss your option of a third course with your faculty advisor. Fees are due after you register for courses. You MUST have a course of study plan signed by your faculty advisor on file by the end of the first semester. We will post an announcement on the Graduate SacCT site when the form is available (under review). A course of study plan will ensure successful progression through the program. Most students follow the Three-Year Plan below, but you have seven years to complete the program http://www.csus.edu/gradstudies/7yeardeadline.htm. Whatever plan you choose, please note that, 1) each course is only offered once a year, and 2) you must satisfy the prerequisite requirements before enrolling in a course, and 3) candidates enrolled in the N500 culminating experience may take a maximum of 3 units concurrent with N500, 6 units total. Good news! The WPG is satisfied by completion of NURS 209 with a B or better grade. Graduate Program SacCT Site. Please read the Graduate Student Handbook on the Graduate Program SacCT site prior to the start of instruction, https://online.csus.edu/webct/entryPageIns.dowebct. If you have any questions, write them down and bring them with you to one of the orientation sessions. We regret some recent changes, such as a change in our background screening, drug and alcohol screening, and document tracking provider are not reflected in the handbook—they will be updated soon! iMEET Tutorial & Equipment. Many of your classes will be moderated on iMEET. Before your first class, we recommend you complete the online and participant orientation and review the reference guides For Participants, http://www.csus.edu/atcs/tools/imeet/handouts-tutorials.stm#participants. Any concerns—ask Dr. Curtis for a quick tutorial at one of the drop-in orientation sessions. Textbooks and Course Materials. We recommend you purchase your textbooks early to avoid long lines. The list of required and recommended books will be available on the Hornet Bookstore website on July 5 (see “Books – Textbooks & Course Materials”), www.hornetbookstore.com. If there are no materials listed, information will be provided by faculty on or before the first day of instruction. Please watch your Graduate Program SacCT site Announcements and Mail for special instructions from faculty. OneCard – Sac State ID. Here is a link to information regarding the Sacramento State, OneCard, http://www.csus.edu/aba/onecard/About_onecard.html. We encouraged you to purchase a OneCard, so you can have full access to all of the services provided at Sac State! Student Professional Liability Insurance. All nursing majors are charged a $20 Student Professional Liability Insurance fee annually each fall while enrolled in the nursing program (the fee is subject to change). Student Financial Services will automatically include this fee with your registration fees after instruction begins. Continuous Enrollment. If you do not enroll in courses, or you take more than one semester off while in the program, you will lose your Catalog rights and you will be required to reapply to the University by the application deadlines provided on the Office of Graduate Studies website. (FNP students – you are considered a continuing student as long as you are enrolled in courses at UC Davis.) 3 of 3
