HEALTH PORTFOLIO PROJECT
This personal portfolio project must include the following items to earn the full 115 points. Also, your items will be graded
on effort, which includes: neatness, creativity, quality and following directions. I have included a minimum amount of
work for each section, but you are encouraged to exceed the minimum amount of effort. This is hopefully a memento that
you will keep for many years to come and I do not need you to include my name or health class anywhere on this
assignment.
Please put this assignment together NEATLY in one of the following ways: Book format that is bounded, 3-ring binder with
sleeves or neatly hole-punched, scrapbook, or large journal. Do not just turn in this assignment loose leafed in a file folder
or in the sides of a folder. (Points will be deducted)
ENJOY learning more about yourself and enjoy this assignment…it is all about YOU!!!
1) INTRODUCTION PAGE: This page is all about you and your interests! Please include a picture of yourself on this
page and feel free to decorate the page in a way that represents who you are. Please include: Name, birthplace,
family, favorite actor & actress, favorite movie, favorite singer, favorite song, song that best describes your
personality, favorite food, favorite comfort food, most treasured possession, best quality, worst quality, something
that makes me see red, greatest love, future plans, best one word that describes me.
2) MOTTOS TO LIVE BY:
Look up motivational quotes (positive and appropriate) and select the ones that inspire you the most to be a better
person. Find a minimum of 5 quotes and NEATLY arrange them on paper. It can be neatly/creatively written, designed
on the computer, or pieced together from a magazine as long as it is neat, colorful and creative. Add a meaningful
background; include pictures, clipart, or anything to make it more meaningful to you! Don’t forget the authors.

3) PERSONAL MISSION STATEMENT:
This is like a personal credo or motto that states what your life is about. It is like the blueprint to your life, stating what
you will try and do life and what is most important to YOU! Even Solon HS has a mission statement that states what we
are trying to accomplish as a school, but here are some examples of teenager mission statements in different formats.
Pick and format that is best for you, and it doesn’t have to be one like the following. Must be original.

I will not underestimate the power of family unity.
I will not neglect a true friend, but I will set aside time for myself as well.
I will cross my bridges as I come to them.
I will begin all challenges with optimism, rather than doubt.
I will always maintain positive self-image, knowing that all my intentions will begin with self-evaluation.

a bad day
NOW…You come up with your own mission statement! Make it neat by using the computer, or creatively writing it out.
Make a neat background and try and make it colorful and neat! (15 points)
4) I SEE THE LIGHT! (2 separate essays)
a) Write about a person that has made the most positive difference in your life. Explain what qualities that person has that
you would like to develop. Give some background on the person and your relationship, and then give support to what they
have done to make such an impact on your life. (Minimally, this should be one well-developed paragraph)
b) Write about a time when you were deeply inspired. This could also be described as a “light bulb moment” where
something important was discovered or became very clear to you. Describe how this has affected you and your attitude
today. (Minimally, this should be one well-developed paragraph)
Make sure these 2 essays are neatly written (if you have REALLY neat handwriting) or typed. Maybe include a picture or a
neat/colorful background to add a special touch.

5) LETTERS OF AFFIRMATION:
Have the people closest to you write a letter of affirmation. These people could include your parent, cousin, aunt/uncle,
sibling, friend, teacher, etc. Please ask at least one family member to complete this task. Ask each person to write a letter
to you answering the question “you are such a special person because…” This letter is personal to you; therefore there will
be no length requirement. Please include a minimum of 3 letters.

6) GOAL SETTING:
You will need to come up with a minimum of 5 short-term and 3 long-term goals for yourself. These goals could include the
following items: school, college, work, extra-curriculars, attitude, health, etc. Since this is Health class, I would like you to
make you include at least one health-related goal. Short-term means you would like to accomplish these within a shorter
period of time (within high school). Long-term means you would like to accomplish these within a longer period of time
(after high school/lifelong). These goals should be very specific and measurable. Your goals should also be challenging
without being unrealistic. Once you set your goals you need to write WHEN that you plan on accomplishing each goal.
Finally, you need to write a plan on HOW you plan on attaining each goal.
For example:
Short term goal #1Goal: I will raise my grade in Health class from a B to an A.
Time: It will be accomplished by the end of Semester I.
How: I will complete all of my homework assignments, take better notes and compare with a friend, as well as study for a
minimum of one hour for each test. I will ask Ms. Davenport what I can do to perform better on my tests and finally I will
spend 20 minutes 3 times per week on my portfolio project until it is completed.

7) THE IMPORTANT THINGS IN LIFE:
Make a collage of the things and people that you admire most in life. Somewhere on this page should be a picture of
yourself. You can use actual photographs, magazine pictures, drawings, words, etc. This page should be full of all the
important things in YOUR life. (One page minimum and no maximum amount of pages!) 15 points
8) EXTRA! EXTRA! READ ALL ABOUT IT!!
Write a feature article from a newspaper or magazine all about YOU! The more real the article looks the better! In this
article, write all about something great that you hope to accomplish in the future. Have the article include how you
achieved your success and use pretend quotes from other people about you, along with your own. It could be from any
magazine or newspaper, but make sure that it is a reasonable goal to attain. Don’t just write about how you won the
lottery and now you are the richest person in the world. Write about how you accomplished one of your attainable longterm goals! You will need to use the computer to type your article and you should include a picture of yourself in the article.
