ACKNOWLEDGEMENT OF POLICY
I have read and understand the Program Details handout for Summer School PE/Health. I understand that the
student enrolled in the class is responsible for completing the Health curriculum on their own time.
I am also aware of the fact that, there will be times in the PE class that the student enrolled in the class will not be in
direct line of sight of the teacher or teaching assistant. I know that the expectations for the student while they are
not directly supervised are the same as if the teacher were supervising them.
I understand that the student will be held accountable for their choices while in class.
