Medical Family Tree Assignment
PART ONE: EVALUATING YOUR OWN PERSONAL HEALTH
Understanding the strengths and weaknesses of your own personal health habits is an
important first step in paving the way to a healthy future.
1. Complete the “Personal Health Assessment.” Answer all questions as best as possible
and total up your score. Attach your completed survey to the back of this project.
2. Complete “Personal Health Assessment SII Reflection” sharing your reactions—
strengths, improvements, and insight—to your results. Submit with final project.
PART TWO: FAMILY MEDICAL TREE DIAGRAM
Knowing your family’s medical history can help you with early screening detection and
prevention of disease. This information can be shared with your family and health care
providers.
1. Getting started (optional): Read “Medical History: Compiling Your Medical Family
Tree” (http://www.mayoclinic.org/healthy-lifestyle/adult-health/in-depth/medicalhistory/art-20044961). This will give you ideas on how to get started, but if you are
struggling, the best way is to just talk to your relatives
2. Please go back as far as you can in your family tree (to at least your grandparents, if
possible), and create a diagram on letter/legal size paper (8 ½” x 11”) that depicts your
family’s medical history, showing the following:





Major (non-communicable) physical and/or mental illness/addictions
Age of onset of major illness (if known)
Cause of death/ age of death (if known)
Squares = Male; Circles = Female (or other design if shown in “Legend”)
Note those who have passed away (i.e. slash, “X,” or other mark)
PART THREE: ANALYSIS OF DIAGRAM
After compiling and organizing your medical data, you will write a report (~2 pages)
analyzing and summarizing your findings. This should be a well-written paper, free of
spelling and grammatical errors. You should utilize good writing techniques—including
logical organization, topic sentences, and transitions. See rubric for detailed criteria.
