BridgeBuilders Ministry Application Process
Thank you for your interest in short-term missions at Wooddale Church. Please follow the instructions below for
completing and submitting an application to participate in Wooddale’s BridgeBuilders Ministry to Guatemala.
Application Instructions
Please complete the Team Member Application electronically. You may type your name on signatures lines or insert an
image of your signature. Please email your completed application to Tim Brands (timothybrands@gmail.com).
Application Requirements
Application requirements include completing and submitting the following by the application deadline:
☐ The Team Member Application completed in its entirety.
☐ The Release of Liability on the last page will need to be printed and notarized.
☐ A $300 non-refundable Deposit.
☐ The Parental Consent (Minors), if a minor will be traveling with only one parent. Note: it is Wooddale
Worldwide’s policy that minors can participate in the BridgeBuilders ministry only with a participating parent.
Wooddale Worldwide reserves the right to deny any person’s application if Wooddale Worldwide determines that person’s
participation would not be in the best interests of the individual, the BridgeBuilders team, Wooddale Church, or our
ministry partners in Guatemala. Wooddale Worldwide reserves sole discretion in approving and denying Team Member
Applications.
Application Deadlines
Airfare and lodging are significant expenses in our budget and the logistics related to airfare and lodging for a large group
can be very complex and time-consuming. Therefore, group reservations for air and ground transportation, lodging, and
meals will be made only for individuals who have fulfilled all of the above Application Requirements by the following
application deadlines:
1. For repeat BridgeBuilders participants: Sunday, October 26, 2015 (Fall Missions Fest)
2. For first-time BridgeBuilders participants: Sunday, November 8, 2015
Applications submitted after the applicable deadline may result in additional costs.

Code of Conduct
Short-term ministry opportunities are about representing Jesus Christ and Wooddale Church to people who may not know
Jesus or Wooddale Church. Thus, as a member of a Wooddale Church short-term ministry opportunity it is expected that
you conduct yourself according to the highest standards of integrity and morality, so as not to compromise the team, the
ministry partners, or the image of Christ. In summary, we ask that you refrain from any behavior that may hinder your
ministry or the ministry of the team for the duration of the trip; and, we ask that you follow these guidelines, recognizing
that should you conduct yourself otherwise – you, the team and the ministry are put at risk. By reading and signing this
Code of Conduct you are agreeing to abide by Wooddale’s guidelines for the duration of your trip. Failure to follow these
guidelines will result in disciplinary action up to and including dismissal from the team, in which case you will travel home
at your own expense.
I will:
1. Make my best effort to attend all team meetings prior to departure, during the trip and post-trip. If unable to attend,
arrangements must be made with team leaders to cover the material presented.
2. Go as a servant of Jesus Christ and adopt that attitude when dealing with fellow team members and people I meet
during the trip.
3. Be flexible in attitude, recognizing that schedules, activities, and assignments often change.
4. Interact with all team members, not just those I knew prior to the trip.
5. Accept and submit to the leadership and authority of the team leader(s) and/or country host(s) and promise to abide
by his or her decisions as they concern this mission trip.
6. Respect advice given by the country host(s) regarding attire, eating and drinking, personal interactions, and
traditions that will help me better understand and assimilate into the local culture.
7. Follow the guidelines provided by my team leader(s) and/or country host(s) regarding gift giving.
8. Be circumspect regarding what could be perceived as flaunting our wealth. It is not uncommon that the jewelry,
cameras, electronics etc. that we take with us on a trip cost more than those we visit may earn in years.
9. Be respectful in regard to photography. We all want to remember our trips with photos. Yet we must respect the
dignity of our hosts. This is especially true of video recording.
10. Ensure that the team leader(s) knows where I am at all times. I will not wander off or follow my own agenda
regarding the use of my time during the trip.
11. Refrain from using tobacco, alcoholic beverages, or illegal drugs at any time while on the mission trip.
12. Refrain from inappropriate public displays of affection with team members and one-on-one social activities with
nationals of the opposite sex.
I have read and understand and agree to the above guidelines for conduct while on my short-term mission trip. I recognize
and agree that in the event that my conduct is considered so unsatisfactory that it jeopardizes the success of the trip, and
that mediation during the trip has failed to correct my behavior, that my participation in the mission trip shall end and I
shall return home immediately at my own expense.

Financial Obligation
This Financial Obligation Form is to assist you in making a plan for raising your funds and meeting your financial
responsibility for the BridgeBuilders ministry.
Trip Costs
The BridgeBuilders trip is typically budgeted at $2,200 - $2,400 per adult and $1,500 - $1,700 per child age 16 and
under. This amount covers all air and ground transportation, in-country meals, lodging, and all ministry expenses. The
exact amount for this year’s trip will be determined and communicated in November. YOU SHOULD START
PREPARING TO SEND YOUR LETTERS BEFORE THE END OF THE YEAR.

Seeking financial and prayer support will stretch your faith, the team, and your community.
Fundraising for your trip is an opportunity to see God’s faithful providence at work.
Most expenses are determined after the fundraising amount is set. As a result, expenses can quickly exceed our
budget. Even if God has already provided full funding for your trip, we encourage you to send out letters
requesting prayer support and additional funds for the general support of this ministry.
People like helping out and contributing to such causes; they WANT to be asked!
Be contagious! Get excited. Invite others to participate and get involved.

Financial Commitment Deadlines
1. Deposit: $300.00 non-refundable deposit is due with the Team Member Application
2. Fundrasing: 90% of the required funds are due two weeks prior to our departure
3. Balance: Any remaining balance needs to be received within two weeks after the team returns. You are
responsible to pay any funds you do not raise. Fulfillment of this obligation is considered for future participation in
the ministry.
4. Updates: Periodic fundraising updates will be made available to you in the weeks prior to the trip.

injury, loss, damage, or accident that might occur during its missions trip to Guatemala for the approximate dates of February 20-29,
