To Be Eligible To Participate In GAP




Choose an intended degree program from
the list of participating programs.
Submit all required admission credentials
with test scores that exempt student from
developmental course work.
Meet with major advisor before August 1.
Have no prior college credit (except credit
acquired through Hall High School)
Continued Participation In GAP



Follow exactly the 8-semester degree plan for
the intended major as outlined in the degree
program contract.
Never change the major/minor program after
the GAP contract is signed.
Have class schedule approved by official
GAP departmental advisor and register for
classes each semester during the EARLY
(priority) registration period.
Continued Participation In GAP
Accept any available course section that
can be accommodated, or notify official
GAP advisor in writing, before the first day
of class that a contract variation and/or
adjustment will be required.
 Be continuously enrolled and complete 3036 semester credit hours of approved
coursework each academic year as
outlined in degree-plan contact.

Actions That Void GAP Participation
Dropping or failing a course
 Withdrawal from University for any reason
 Non-payment of tuition or fees
 Disciplinary actions


appointments with the students. Below are a few items to



the student’s admissions application information

The ‘second block’ of SAAADMS will show the Checklist
summary, which includes the ACT 1014 (GAP)
agreement as a checklist item
BANNER-SOATEST

SOATEST (Student Test Information Form) shows ACT, SAT,
and CAAP scores that have been entered into BANNER
BANNER-SWAADVS

SWAADVS (Advising Flag Form) is used to
review catalog term, view GAP status, and lift
advising flags


Acceptance/Waiver Form
Each student that initially chooses to participate in GAP during the online
application process MUST complete this form (either accepting or waiving
participation). If a student is deemed ineligible, he or she must complete the form
during the initial advising appointment in University College or major department.

Violation of Contract Form
This form must be
submitted to The Office of
Admissions if a student
violates his or her contract
by committing an action of
voidance as noted on the
GAP Participation
Acceptance Form.
Submission of this form
relieves the advisor of all
duties associated to that
particular student.
Course Schedule Conflict Form
A student must submit this form to his
or her GAP departmental advisor if
issues arise during the registration
process and the student cannot enroll
in a course required for that semester.
Also, this form should be used if the
student is dropped from a particular
course section due to circumstances
beyond both the student’s and the
University’s control (I.e., cancelled
section, time change).
The form must be approved by
the GAP advisor and a copy
should be placed in the
student’s advising file.

Test Score Interpretation Checklist
This form is used to interpret
students’ standardized test
scores and assess GAP
participation eligibility.
If a student’s score on a
particular test section requires
him or her to enroll in
developmental coursework,
then the student is
automatically ineligible and
the student’s status must be
recoded in BANNER.
Undergraduate Student
Advisement Record
This form is to be used by the
departmental GAP advisor
during each semester’s
advising session. Similar to
University College’s
procedure, the advisor keeps
a copy of the form in the
student’s folder. The student
and the student’s major
department receive copies as
well.
FERPA Consent Form
In accordance with The Family
Educational Rights and Privacy
Act (FERPA), this form must be
completed at an advising
session if anyone besides the
student attends that advising
session. The form serves as
written release of education
records to whomever (parent,
spouse, friend, etc.) is in
attendance.
NOTE: Once this form is submitted to the GAP advisor, the person(s) named on the
form will be able to access that particular student’s educational records. Access to the
student’s records must be revoked by the student in writing.

freshman during the initial
