General Job Description
Processes, reviews, and receives patient data and record forms from particular therapeutic groups and outside
investigators. Provides accurate, timely, and consistent clinical data to the medical department and other groups.
Responsible for data management plans including data preparation, data validation activities, etc. Follows Good
Clinical Practices (GCP) data-handling procedures and guidelines. Requires knowledge of drug and disease
terminology. May manage service providers that perform these activities. Participates in the review of clinical
research protocols, reports and statistical analysis plans. Carries out duties and responsibilities consistent with
local laws and regulations. Requires a Bachelors degree from an accredited college in a related discipline, or
equivalent experience/combined education, with 3 years of professional experience; or 1 year of professional
experience with a related Masters degree.
Specific Job Description
Travel required on an as needed basis (up to 95%).Work schedule fluctuates greatly. The work locations
for this position will vary and require travel in the Continental United States (CONUS) and, possibly,
Outside the Continental United States (OCONUS). The Medical Data Abstractor (MDA) is responsible for
reviewing paper and electronic medical charts, interpreting and analyzing medical record documentation,
and extrapolating pertinent health information. Upon receipt of documentation, the MDA reviews and
verifies component parts of inpatient and outpatient medical records and determines that all the final
diagnoses and procedures, are applicable. The MDA will collect data from all charts completed by an
organization, using pre-defined data collection metrics and accurately enter the data into a web-based
data collection tool/database. The MDA is responsible for researching all available resources on missing,
conflicting or ambiguous information in the medical record. Keep abreast of continually changing
information, concepts and requirements in the field of medical review, coding and documentation and
healthcare quality. Apply skilled judgment in determining that deviations from these guidelines may
indicate an omission or inaccuracy within the patient’s medical record. Present written and oral findings
with explanations to organization’s leadership. Work collaboratively with a wide range of healthcare
professionals including providers, medical records personnel and other healthcare leaders.
Duties and Responsibilities of this Level

Duties will include reviewing and screening paper and electronic medical charts, interpreting and
analyzing medical record documentation, and extrapolating pertinent health information. Upon receipt of
documentation, the abstractor reviews and verifies component parts of inpatient and outpatient medical
records and determines that all the final diagnoses and procedures, are applicable.

Collects data from all charts completed by an organization, using pre-defined sets of measures and
accurately enters the data directly into an electronic health record data collection database


Keeps abreast of continually changing information, concepts and requirements in the field of medical
review, coding and documentation and healthcare quality.


Presents written and oral findings with explanations to organization’s leadership.

Works collaboratively with a wide range of healthcare professionals including providers, medical records

Create an abstract summarizing such clinical data as diseases, therapies, surgeries, lab tests and
pharmaceuticals

Maintain a thorough understanding of anatomy and physiology, medical terminology, disease processes
and surgical techniques through participation in continuing education programs to effectively apply ICD-9CM and CPT-4 coding guidelines to inpatient and outpatient diagnoses and procedures

Maintain a thorough understanding of medical record practices, standards, regulations, The Joint
Commission (TJC), Health Care/Finance Administration (HCFA), etc.

Assist with special projects as required.
Required Qualifications at this Level
Licensed RN, or RHIA/RHIT, (with certification maintained) is required. Bachelor degree from an accredited college
or equivalent experience/combined education. Minimum of 5 years of professional experience or 3 year of
professional experience with a related Masters. Experience in an inpatient hospital setting is required. Computer
experience using Word, Excel, and data entry programs. Customer oriented. Excellent interpersonal, oral, and
written communication skills. Excellent organization and analytical skills. Must be able to work independently to
meet deadlines. Accepts and performs other related duties and responsibilities as required. Will be required to
obtain/maintain a Public Trust investigation.
Degrees, Licensure, and/or Certification: Registered Health Information Administrator (RHIA) or Registered Health
Information Technician (RHIT) or Registered Nurse (RN)
Knowledge, Skills, and Abilities:

Military and/or VHA medical record abstraction experience
 ORYX medical record abstraction
 Advanced ICD-9-CM & CPT-4 coding conventions

Anatomy and Physiology

Medical Terminology

Extensive DRG/APC reimbursement knowledge

Effective written and verbal communication skills

