Dear Dr. Andoy:
We are students in Ms. Liza Ayo System Analysis and Design class and
are engaged in a system project on the matter involving students’ violation. As
part of the requirement to our project, we are writing to request permission to
acquire information from the Student Affairs Office mainly on the monitoring and
updating of students' violation. We would like to know the operations involve in
recording students violation. if possible, we would like to take pictures of each
process undergone. We also would request permission to have a copy of the
forms from the Student Affairs Office used for recording students' violation and
also the cost involve in making use of the current method of keeping information
of students with violation.
