The FAIM scholarship was created for the purpose of recognizing outstanding students who have participated and succeeded in the upper levels of the AIM program. This program is for students who have completed at least level 6 in the AIM program and who have applied and been accepted as FAIM students. Scholarship applicants must have participated actively in the FAIM program for at least two years to apply for the scholarships. There will be 10 scholarships of $100 awarded yearly. These scholarships are based on many areas. Students must be outstanding musicians as well as musicians who use their abilities to enhance their communities. These scholarships will be awarded to the top ten applicants as decided by the AIM Committee. The awards will be given at the UMTA State

Please answer the following questions as completely as possible on separate sheets of paper. Please submit your typed answers with this form as the cover sheet.

1.

Please list all of the years you have participated in AIM and your cumulative scores.

2.

Please list ways you have participated in the FAIM program. List performances and services performed.

3.

List all of your achievements associated with your private music study. List first your accomplishments as related to the piano and then other instruments.

4.

List your achievements in music as related to your school music programs, if applicable.

5.

List your services to the community as related to your musical abilities.

6.

Write a paragraph or two describing why you feel you represent the top piano students in the state of Utah and why you should receive this scholarship award. Give credit to those who have helped you along the way.

7.

Submit two letters of recommendation. One should be from your private piano instructor and the other should be from one who knows about your musical abilities and services.

8.

Submit no larger than a 5 x 7 photo of yourself with this application. The photos will not be returned.
