Greetings from Indonesia!
Please find necessary documents related to your plan to study at Petra Christian University, Surabaya –
Indonesia.
In general, there are 4 main files that you will need to review:
File 1) Form of Admission Application for International Student F.03.P.BAKP.01
File 2) Study Permit Application to be filled by student (partially filled in by Petra)
File 3A) 6 months program_STATEMENT LETTER on visa arrangement
File 3B) 1 year program_STATEMENT LETTER on visa arrangement
File 1) Form of Admission Application for International Student F.03.P.BAKP.01
This will serve as your application form to be international student of Petra Christian
University. Please read it very carefully and complete everything that is requested in the form.
You have to provide the English translation for your supporting documents which are written in
language other than English.
To fill in the form, please TYPE your data or use CAPITAL LETTERS and sign it. You will
find list of documents that you have to submit to Petra in order to support your Study Permit
application. See File 2. Partially filled in by Petra_Study Permit Application to be filled by student.
File 2) Study Permit Application to be filled by student (partially filled in by Petra)
Study Permit is an important legal document issued by Bureau of Planning and
International Cooperation, Department of National Education in Jakarta – Indonesia. Study Permit
Application consists of 4 kinds of forms, as follows:
1. APPLICATION FOR ADMISSION
Please pay attention to section:
A. University and Faculty or program applied for:
You have to check this information with your academic supervisor in INHOLLAND or with
your host department at Petra.
B. How long and when you start to study In Indonesia?
We will let you know about your expected arrival date.
2. CURRICULUM VITAE
On the Curriculum Vitae form you will be asked about your permanent and present
address. The present address means the place where you reside currently, while the
permanent address is more about your emergency contact where in any emergency case
you can always be reached or somebody will be there to help mediate the communication
with you (mostly by letter or phone call). For example, a student is renting an apartment

in Surabaya because this person is studying at Petra Christian University, but he originally
comes from Bali, and he used to live in Bali with his parents before he goes to Surabaya.
So, his address in Surabaya is his present address and his address in Bali is his permanent
address. However, it's also possible that a person has the same present and permanent
address.
3. DECLARATION AND CERTIFICATION OF FINANCE FOR STUDYING AT PETRA CHRISTIAN
UNIVERSITY, SURABAYA – EAST JAVA, INDONESIA
This form is about the sponsor of your funding. In Indonesia and some other countries,
the student's parents will become the one who pays the tuition fee or other financial
matters of the student. Therefore, the guarantor for this page will be one of your parents.
I don't know in what way it is different with your condition. So, this form should be filled
in by the person who can appear as the sponsor of your funding.
4. WRITTEN OATH
There is a series of arrangement that has to be done to get this and Petra as your host
institution will help you to prepare this. It will take 1.5 (one and a half) months to arrange
for this Study Permit, therefore, please make sure that you have provided Petra with the
correct and complete documents by the due date.
5. SPECIFICATION OF PHOTO
Please follow these instructions for your photo’s specification:
 The photos must be 4 cm x 6 cm (4 pieces) and 3 cm x 4 cm (4 pieces). Identical
color photos are required.
 The photos should be in color with a medium red background (please check file:
Red background for photo.jpg for information on the kind of RED background
that this photo requires). This means that the RED color should not be so bright as
to reflect light and affect the clarity of the picture and not so dark as to cause the
details of the face to be lost in a dark background.
 Your face should be in the center of the photos and occupy about 75% of the
photo area. Make sure the photo presents the full head from the top of the hair
to the bottom of the chin and should include shoulders, therefore, you have to
wear formal clothes for this photo purpose.
 The person in the photo should have a neutral expression, be facing straight into
the camera and doesn’t wear glasses or color contact lens.
 The photos should not be more than six months old.
 The photos should be printed on photo quality paper.
File 3A) 6 months program_STATEMENT LETTER on visa arrangement
Students who are going to stay and study at Petra for 6 months, they should use VISA
KUNJUNGAN SOSIAL BUDAYA (SOCIO CULTURAL VISA), visa index: B 211. Study Permit and
Socio Cultural Visa are a pair of initial documents that you have to arrange before your depart for
Indonesia. More explanation on this will be provided in the statement letter.
File 3B) 1 year program_STATEMENT LETTER on visa arrangement
Students who are going to stay and study at Petra for 1 year, they should use VISA
TINGGAL TERBATAS / VITAS (TEMPORARY RESIDENT PERMIT VISA), visa index: C 315 or C 316.
Study Permit and Temporary Resident Permit Visa are a pair of initial documents that you have
to arrange before your depart for Indonesia. More explanation on this will be provided in the
statement letter.

There are still other documents that you have to take care of within 24 hours after you arrive in
Indonesia, these documents will be explained in detail in either File 3A or 3B. Please send the original
signed version of either FILE 3A or 3B to Petra and keep a copy for your personal reference.
