***TSB announcement: DALLAS - Delivering Assisted Living Lifestyles at Scale – Dates Announced
A total investment of up to £23 million is being made in the UK-wide DALLAS programme –
Delivering Assisted Living Lifestyles at Scale. This comprises an £18m investment by the Technology
Strategy Board and the National Institute for Health Research, with a further £5m contribution from
the Scottish Government, Highlands and Islands Enterprise and Scottish Enterprise.
DALLAS will establish three to five communities of 10,000 people each or more across the UK, of
which one will be in Scotland. These will show how assisted living technologies and services can be
used to promote wellbeing, and provide top quality health and care, enabling people to live
independently – including a preventative approach.
We aim to unlock new markets in social innovation, service innovation and wellness, enabled by
technology, and show that technologies and services can be made available at a sufficient scale and
cost to enable independent living. The competition will help to grow the assisted living sector and
position UK companies to take advantage of increasing global demand for assisted living goods and
services.
This is an open competition commencing on 7 June 2011, with successful projects attracting 100% of
public funding and lasting up to 36 months. Applicants must register by 20 July 2011 with a first
stage submission deadline of 27 July 2011.

In a very interesting new development, Pfizer is launching a home-based clinical drug trial using
social media for online enrolment, home delivery of materials and remote condition monitoring
using mobile phone apps/home computers.

***Headline 2 - Teledermatology should not replace total body examination in patients at risk for
skin cancer, researchers say
Yale University researchers have recently published a cohort study of 400 Veterans Affairs patients
who had used telemedicine services and had been referred to in-person consultations with
dermatologists at two academic medical centers and six community-based facilities for suspicious
skin lesions between 2006 and 2009. Approximately 56 percent of the 400 suspicious lesions were
immediately ruled out by dermatologists as nonmalignant and did not require biopsy. However, a
total of 149 skin cancers that had been missed by the referring clinician were identified on other
parts of the body. About 22 percent of these proved to be malignant and required biopsies.
Most of the malignancies that were identified were basal cell carcinomas. About one-quarter of
them were squamous cell carcinomas, and five cases proved to be melanomas. This prompted
concern among the researchers, who pointed out that if the dermatologists had looked no further
beyond the flagged index lesions, they would not have found these diseases.
The scientists said that the assessment of a single, specific lesion of concern is typical in
teledermatology. They concluded that this could lead to the "underdiagnosis of clinically significant
lesions that are not appreciated by the referring physician." As a result, they emphasized that
teledermatology should not be relied upon as a substitute for total body examination of patients in
whom skin cancer is suspected.

***Headline 4 - Warning: Mobile medical apps may be hazardous to your health
A recent study that evaluated 47 iPhone applications aimed at helping smokers to reduce their
tobacco use found that very few of the applications were evidence-based. The study performed at
George Washington University concluded that the iPhone applications for smoking cessation sold in
2009 “rarely adhered to established guidelines.” Very few of the iPhone applications recommended
treatments that have been proven effective. The implication of the study was that by failing to
follow scientific guidance on best practices, the iPhone applications may be ineffective in helping
consumers.

***Headline 5 - Are mobile apps the key to reducing hospital readmissions? (USA)
The US Patient Protection and Affordable Care Act (PPACA), which goes into effect next year, has
established benchmarks for hospital readmission rates. Initially, it will track readmission rates for
congestive heart failure, heart attack, and pneumonia. Hospitals that don’t meet the benchmarks
will lose 1% of its income. In 2014 the government will track three more conditions with high
readmission rates: Chronic obstructive pulmonary disease (COPD), coronary artery bypass surgery,
and coronary angioplasty, Penalties for high readmission rates will increase to 3%.
According to the article, people are willing to use mobile apps and take a more active role in
managing health. It refers to a Journal of Medical Internet Research study of more than 100 heartfailure patients in Toronto that showed they were comfortable using mobile apps to manage their
condition.
Even though mobile health has the potential to revolutionize healthcare, it also presents some major
challenges, such as increasing clinical workloads and raising important questions about liability. For
example, if someone buys a health app and experiences a major health issue as a result of using the
app, who is liable—the app store, the developer, the carrier, or all of the above?

Some reported outcomes from a recent study include 

A readmitted patient is charged an average $5,600 for new hospital treatment. Monitoring a
patient by phone costs about $230 per month, including staff time. The machines cost $3,200
apiece.
About 660 patients have taken part in the program. 137 were not readmitted in the reporting
period. Potential savings to insurers - about $767,000.
A few chronic health problems account for the majority of readmissions. Among the most
common are chronic lung disease, heart failure and complications after heart attacks.
Generally, 20 percent to 25 percent of people will usually have to go back to the hospital within
30 days of discharge according to the U.S. Department of Health and Human Services.
Congestive heart failure readmissions average about 24 percent but are about 12 percent for
those in the program.

Ambulance services have been too focused on response times and not enough on patient care,
according to the National Audit Office (NAO). There are wide variations in the costs per incidents

A recent conference brought together a new generation of self-trackers – people who wish to collect
as much data about themselves as possible to help them make decisions about health, productivity
etc. At the heart of the approach is a smart phone sometimes supported by sensors and apps to
track food intake, fitness, mood etc. Several commercial wearable monitors employ accelerometers
to track the wearer's movement, pairing that with specialised algorithms to calculate calories
burned. Data is automatically uploaded to the internet, allowing users to track their progress and
compete against each other for the most steps or highest activity levels.

An eight-week rehabilitation at home programme for patients recovering from critical illness was
found to have no effect on the rate of improvement, according to researchers. A randomised trial of
195 patients in Australia assessed physical ability after a support programme involving a series of
home visits and follow-up phone calls, supported by a printed exercise manual. Those assessed had
all been in intensive care for at least 48 hours and had been on ventilators for at least a day. The
programme did not increase the underlying rate of recovery compared with a control group.

incident if they are involved in their care, a study has found. American researchers surveyed 2,025

A new WHO report ‘mHealth: New Horizons for Health through Mobile Technologies’ looks at data
from 112 countries by 14 mHealth activity types and concludes that:

83% of governments surveyed report at least one use of mobile phones to support health
activities in their country
the majority of these projects are still in pilot phase
Two-thirds of countries reported between one and three mHealth activities, yet only 12%
have even evaluated their efforts in this area
The four most frequently reported initiatives were health call centres (59%), emergency free
phone services (55%), managing emergencies and disasters (54%), and mobile telemedicine
(49%)
The WHO report considers that the UK is a leader in mHealth because of its progress with text
message appointment reminders and the National Health Service's direct healthcare phone helpline.

FDA says it's looking for "greater regulatory clarity."
As smartphone apps become increasingly more sophisticated there is concern that some devices and
applications may do more harm than good, especially if they don't meet basic standards of
reliability, safety, and security.
