The golfer must be a high school senior.
He must have a minimum GPA of 3.5 on a 4.0 scale or the equivalent. Please convert to 4.0.
He must have a minimum combined math and verbal SAT score of 1000, or ACT equivalent.
He must participate in at least 50% of this team’s 2008 varsity golf matches
His seven semester transcript must be submitted with this nomination form.
His nomination form must be signed by both his principal and his coach.
His coach must be a current member of the Indiana High School Golf Coaches Association.
