I hereby declare that I:
… have never been through any form of marriage or civil
partnership, be it in the United Kingdom or elsewhere. I
understand that this includes both Religious and Civil
ceremonies whether they have taken place indoors or out of
doors, in the presence of any religious or civil minister.
I also affirm that no other relationship has been recognised
or “blessed” in any way and that I am therefore free to
marry in the Catholic Church.
I make this statement freely and fully aware of the
implications of the statement I am making.
