Pet Grooming Agreement Form
I do hereby entrust my pet(s) to Rover Done Over for the purpose of grooming my pet(s) understanding that all
steps will be taken to ensure the safety of your pet. I do understand that if my pet is wiggly or doesn’t remain
still, that accidents can happen such as nicks from clippers or scissors. I am aware that clipper and brush irritation
could be the result of the neglect of my pet’s coat prior to grooming. In the event that my pet(s) coat is matted
there are two options.

Rover Done Over will de-mat the pet at $1 per minute (minimum of $15) in addition to regular groom fee.
My pet will be shaved down. (There may be an additional fee to de-mat the ears and tail)
I understand that Rover Done Over has the right to refuse service in the event of a pet that cannot be safely
handled. The use of muzzles, slings, straps etc. are standard and acceptable safety measures. Grooming services
may be stopped mid-groom for the safety of either the pet or the groomer. I understand that payment will still be
due for the portion of grooming that is completed. Rover Done Over has the right to refuse service should a pet
be deemed unsafe to handle.
Cancellation Policy
In the event that I need to cancel my appointment: I understand that I must notify Rover Done Over at least 24
hours prior to my scheduled appointment. You may leave a message on the voice mail. I am aware that a $35 fee
will be added to my next grooming services if I fail to cancel within 24 hours. I also understand that if Rover Done
Over comes to my home and no one is home a $35 fee will be charged.
Very rarely it may be a possible that Rover Done Over may need to cancel an appointment due to inclement
weather and road conditions that may make driving unsafe or possible equipment failure. I understand that every
effort will be made to contact me to reschedule.
Vaccinations
I understand that all my animals must be current on their rabies vaccination. I will provide proof of vaccination
including the vet contact information and records will be confirmed with the veterinarian's office prior to the first
visit and updated annually.
In the event of an emergency, I authorize Rover Done Over to act as my agent regarding the caretaking,
transportation, or securing of veterinary medical attention. I also understand that in this event that I would be
responsible for any assumed costs. Any/all damages, loss or claim shall include but not limited to death, injury, or
shock.
I have read and understand my rights and obligations as written in this agreement for the services of pet
grooming through Rover Done Over.
