A+ Tutor Evaluation Form
Teachers please conduct two (2) evaluations with your assigned tutor(s) at the mid-semester and at the
end of the semester. Part of the evaluation process for the tutor must include quality feedback from a
facilitating teacher in the form of a conference to discuss the evaluation. Please complete the evaluation
