specifications below will not be accepted; they will be returned to the student and not evaluated.
You may download a digital copy of this form from the National Honor Society (NHS) website, but you
MUST turn in a printed, signed copy to the counseling office. We cannot accept emailed forms.

Type cannot be smaller than 12-point with a minimum line spacing of 1.5 lines.
DUE: Friday, March 14, 2014, at 3:30 p.m. in the counseling office.
Forms will not be accepted after 3:30 p.m. on Friday, March 14th – NO EXCEPTIONS.
What is required to become a NHS member?
Admission and continued membership into the NHS is an honor and must be earned.
Members must:
 Agree to complete 8 hours of tutoring (6 weeks) each semester through Learning Lab
partnership.
 Agree to participate in 2 service projects each semester.
 Agree to follow school code relative to academic honesty, school responsibilities and club /
organization behavior code.
 Have and maintain a minimum numeric average of 92 (no rounding).
 Attend all NHS meetings and mandatory events.
How are the candidate’s leadership and character evaluated?
Students are required to submit a complete candidate packet with resume and the required essays. Next, a
Faculty Selection Council, made up of staff members from each department, meet to review the candidate’s
information sheets, transcript, and teacher comments.
The NHS sponsors and the NHS executive board members do not participate in nor do they influence the
selection process in any way.
Applicants will have ten (10) days from the listed Notification Date below to request a meeting with the
advisers to review the decisions of the Faculty Council. For 2014 the date is Friday, April 4, 2014.

Double space each essay, including the question being addressed at the top of each essay. Please
limit each essay response to a maximum of 250 words.
A. What attribute do you most admire about your character & what attribute do you least admire
about your character? Provide at least two anecdotes about yourself that best illustrate your
CHARACTER considering the questions above. Note: DO NOT include your name in the essay,
but personal stories or characteristics are acceptable.
B. Describe a skill, trait or belief that distinguishes you from others.
C. Describe how something you have done has made a difference in someone’s life.
