MODEL RELEASE:
For good and valuable consideration, the receipt and legal sufficiency of which is hereby
acknowledged,
I________________________________________________(Model”) hereby grant Flight to the North Pole
Event Photographer/Videographer, the Photographer/Videographer’s assigns and those
person acting with the Photographer/Videographer’s authority and permission, the right to
take photographs/videos and other still and/or moving images, (“the Content”).
I hereby agree that all rights in and to the Content, including the copyright, are and shall
remain the sole property of the Photographer/Videographer, free and clear from any claims
by me or anyone acting on my behalf. The Photographer/Videographer rights include, but
are not limited to, the rights to: Use and publish the Content; Combine the Content with
textual matter and/or with other pictures and/or media and use the Content for illustration,
promotion, art, editorial, advertising, trade and publishing.
I hereby release, discharge and agree to hold harmless the Photographer/Videographer, the
Photographer/Videographer’s heirs, legal representatives and assigns, and all persons
acting under the Photographer/Videographer’s authority or those for whom he/she is
acting, from any liability by virtue of any use of the Content.
IN CONSIDERATION of the terms and conditions pertaining to Sun Country Airlines’ Flight
to the North Pole (“Event”), and to preserve the undersigned’s eligibility to participate in
the Event, the undersigned hereby acknowledges and agrees, for his/her personal
representatives or anyone who might claim on his/her behalf, that he/she shall not sue, and
hereby releases, waives and forever discharges MN Airlines, LLC d/b/a Sun Country Airlines
and all other organizations associated with the Event, and all their respective affiliates,
agents, employees, representatives, assigns or anyone acting for or on their behalf
(“Released Parties”), from any and all claims, demands, actions or causes of action, losses or
liabilities of whatever kind or nature whatsoever, foreseen or unforeseen, known or
unknown, either in law or equity, arising from or by reason of any personal injury, death or
property damage or any other loss, of any kind or nature whatsoever, arising from or in
connection with receipt and operation of the contest prize given in connection with the Sun
Country Airlines Flight to the North Pole.
I warrant and represent that I am the model/father/mother/guardian (circle one) of:
__________________________________________, the model. I have read the above authorization,
release and agreement, prior to its execution and I am fully familiar with the contents
thereof. This release shall be binding upon me and my heirs, legal representatives and
assigns.
