Employee Assistance Program (EAP)

EAP benefits are available to your entire family. Coverage includes work-life programs, self-help workshops, counseling sessions, and much more.

Flexible Spending Accounts (FSAs)

You can participate in the Health Care and/or Dependent

Care FSA and pay for your eligible expenses with pretax dollars. The annual amount you elect to contribute to the

FSA is deducted from your pay in equal installments each pay period—before taxes are deducted—and put into an account. When you incur eligible expenses, you submit a claim for reimbursement from your account.

What’s in Store for You at Zappos.com…

Medical

You can choose from two quality medical options:

• CIGNA Open Access Plus Plan–Standard

• CIGNA Open Access Plus Plan–Buy-Up

Both options provide you with referral-free access to health care providers, a 24-hour health information line, wellness programs, and prescription drug coverage.

Dental

The Delta Dental PPO is our dental plan. This plan gives you the freedom to visit any licensed dental provider each time you need care. Diagnostic and preventive services are covered at 100%, up to the plan allowance. Also, there’s an orthodontia benefit for both children and adults.

Vision

Our vision benefits are offered through Vision Service Plan

(VSP). Exams and materials are covered at 100%, up to the plan allowance, after a $10 (exam) and $25 (material, if necessary) copay when you receive services or supplies from a VSP network provider.

Life and AD&D

As a Zappos.com employee, you automatically receive both basic life and AD&D coverage in an amount equal to your annual earnings, up to plan maximum. You may also purchase supplemental coverage for yourself, and dependent coverage for your spouse and children.

Disability

Disability benefits replace a portion of your income if you are disabled and can’t work. The STD benefit, which ranges from 60% to 100% of your weekly salary (up to $2,309 per week), depends on your length of service with Zappos.com.

LTD benefits of 60% of monthly earnings (up to $10,000/ month) begin on the 91st day of disability.

401(k)

If you meet eligibility requirements, you can contribute from 1% to 50% of your annual compensation (up to the

IRS maximum) to the 401(k) plan on a pretax basis. Fidelity is our plan’s trustee.

Objective: Design visual communications that illustrate the company culture and camaraderie that Zappos employees share. Communicate employee benefits and give explanations of all healthcare terms.

Create a highlights brochure and recruiting pamphlet that are as fun as the people of Zappos. Include lots of great employee photos.

If the Shoe Fits…

You are eligible to participate in the Zappos.com benefits program if you are a full-time regular employee working a minimum of 30 hours per week. Your benefits begin on the first of the month following 30 days of full-time regular employment.

You may also enroll the following dependents for coverage:

■ Your spouse or domestic partner

■ Your or your domestic partner’s unmarried children to age 19 (or to age 24 if full-time students)

■ Your dependent children of any age who are incapable of selfsupport because of mental or physical disability.

Shop for Your Benefits

Shopping for your benefits is as easy as shopping for a new pair of shoes on Zappos.com. Simply review your benefit options, consider the family members you want to cover, and complete and return an enrollment form to Human Resources during the open enrollment period or within 30 days of your eligibility date.

If you experience a qualified family status change during the year, contact Human Resources to make changes to your coverage within 30 days of the event constituting a qualified family status change.

Qualified family status changes include the following:

■ Marriage or divorce

■ Birth, adoption, or custody change of an eligible dependent

■ Beginning or ending of your spouse’s employment

■ Beginning of your or your spouse’s unpaid leave of absence

■ Your or your spouse’s change in employment from part-time to fulltime or full-time to part-time.

If you do not contact Human Resources within 30 days following your qualified status change, you will not be permitted to modify your coverage until the next open enrollment period.
