Transfer/Transient/Former Students
 Submit either a paper or an electronic application (www.applytexas.org), NOT BOTH. We encourage you to apply online.
 A “transient” student is one who enrolls temporarily for a single course or one term only.
 Majors and codes are available on the Internet at www.applytexas.org and on institution websites.
Asian: A person having origins in any of the original
peoples of the Far East, Southeast Asia, or the Indian
subcontinent including, for example, Cambodia, China,
India, Japan, Korea, Malaysia, Pakistan, the Philippine
Islands, Thailand, and Vietnam.
American Indian or Alaskan Native: A person having
origins in any of the original peoples of North and South
America (including Central America), and who maintains
tribal affiliation or community attachment.
Native Hawaiian or Other Pacific Islander: A person
having origins in any of the original peoples of Hawaii,
Guam, Samoa, or other Pacific Islands.
Check Fall, Spring, Summer I, or Summer II, and enter the
year you plan to begin attending. “Mini or short semester”
may not apply to all universities.
Check if you are applying as a transfer student, transient
student, or former (re-entry) student. Enter the appropriate

Part I. Biographical Information
1. Enter your Social Security number. The Social Security
number is important to match the application with your
grade transcript and test scores. The Social Security
number is required to give (disburse) financial aid.
NOTE: While no specific legal authority requires you to
list your Social Security number for the purpose of
applying for admission to the university, not providing the
information will hold up further processes, such as
financial aid.

Enter your date of birth (month, day, year). Do not leave
blank; this information is required.
3.
Enter your full, legal name: last, first, and middle. Do not
use nicknames or abbreviations because this information
will be used for your official record if you enroll. Use
your full, legal name on all documents sent to the
institution to which you are applying.
4.
Designate whether you are male or female.
6.
Enter the city, state, and country where you were born.
7.
This question is designed to identify your ethnicity and
race. (a) Indicate whether or not your ethnicity is
Hispanic or Latino. Check “yes” if you are a person of
Cuban, Mexican, Puerto Rican, South or Central
American, or other Spanish culture or origin, regardless of
race. Check “no” if you are not. (b) Select the racial
category or categories with which you most closely
identify. Check as many as apply:
White: A person having origins in any of the original
peoples of Europe, the Middle East, or North Africa.
Black or African American: A person having origins in
any of the black racial groups of Africa.
Answer the questions regarding your citizenship status.
If you have one, be sure to include with your application
a copy of your permanent resident card and/or Notice of
Action from the USCIS.

Indicate if you are connected to the military in any way
by checking all categories that apply to you. If none
apply, leave the question blank.
10. Enter your mailing/permanent mailing address and phone
numbers. Use the Texas county codes and the two-letter
state abbreviations listed on pages xvi and xvii. Use the
county code if your permanent address is in Texas; if it is
not, you do not need to provide a code.
11. Enter your physical address and phone numbers only if
they are different from your permanent mailing address.
12. Enter the name, address, phone number, and E-mail of an
emergency contact person.
13. Enter your E-mail address. The email address provided
may be used to communicate important information to
you regarding your application.
Enter other names, if any, under which your transcripts or
other credentials may be listed. If you attended school
using a different name or took a standardized college
entrance exam using a different name, please list name(s).

fluently and for what length of time you have spoken

15. Indicate the highest level of your parents’ or courtappointed legal guardians’ educational background.
16. Indicate your family’s gross income, including both taxed
and untaxed income, for the most recent tax year.

out the application, leaving item #17 blank, and make
copies. Then enter the name of each university to which
you are applying. List your first and second choice of
major and the appropriate major code(s)/abbreviations for

29. Indicate the date you took or plan to take one of the
following tests for international students: the TOEFL
(Test of English as a Foreign Language) or IELTS
(International English Language Testing System) test.

18. Enter the complete name of the high school from which
you graduated, as well as the city and state where it is
located. Enter the month and year you graduated.
Indicate if you were home-schooled. Have an official
transcript sent to each university to which you apply.
Residency Information
30. – 34. Answer all questions that apply to you. Instructions
are noted throughout the questions.
35. Write down any additional information that would help
your college or university classify you as a Texas
resident.
19. If you did not or will not graduate from high school,
indicate whether or not you have a GED, whether it is the
English or Spanish version, the month and year you
completed the GED, and in what state you received it.
36. Read each statement carefully. Sign your name and
include the date to indicate you understand and agree with
each statement.
20. Indicate whether you plan to pursue a pre-professional
program (e.g., pre-law, pre-medicine, nursing, pre-dental,
pre-veterinary, physical therapy, etc.).
Part II. Additional Information
See pages ii through v to determine if the university(s) to
which you are applying requires this section. If it is required,
provide complete information on employment, internships,
and/or summer activities you have had that indicate your
special interests, abilities, challenges, or achievements. You
may submit a résumé in addition to this list. Attach it to your
application and make a note that you have done so. Spell out
names of organizations, clubs, honors, awards, etc.; do not use
acronyms.
21. Check “Yes” if you plan to seek a teaching certificate in
addition to your degree and “No” if you do not. If “Yes,”
indicate if you will be seeking elementary level (Early
Childhood-Grade 4), middle/junior high level (Grades 48); high school level (Grades 8-12); or all-level
certification.
22. List all college or university courses in which you are
presently enrolled, the institution you are attending, the
department abbreviation, course numbers, dates taken,
and course credit you expect to receive for these courses.
Essays
Most universities in Texas do not require essays as part of the
admission process. Check the list on pages ii through v or the
web site at www.applytexas.org. Be sure to include your
name and Social Security number on each of your essay pages
and enclose your essays with your application. If you apply
electronically, also include on your essays the web
identification number you are given.
23. List all colleges or universities you have attended.
Include their locations, the dates you were enrolled,
any degrees you received, and the date the degree was
conferred. If in Texas, include the school codes (see
pages xiv and xv). Have an official transcript sent to each
university to which you apply from each college or
university you have attended.
24. Indicate whether or not you will have earned a bachelor’s
or associate’s degree by the time you plan to enroll. List
the college or university from which you have received or
will receive the highest degree and the date conferred.
Application Fee
Application fees are listed on pages ii-v. Include the student’s
name and Application ID number on your check or money
order. Check the website of the institution(s) to which you are
applying to see if credit card payment of the application fee is
also accepted. Students with financial need should check with
their high school counselor or the institution(s) to which they
are applying for information concerning a possible waiver of
the application fee (i.e., college application fee waiver).
25. If you are transferring from a Texas community college,
indicate whether or not you would like the university to
which you are transferring to share your transcript with
the community college from which you are transferring
and participate in a reverse transfer arrangement, if
available, in order to earn an associate’s degree.
26. Indicate whether or not you graduated from high school in
the top 10 percent of your class within the last two years.

Procedure for Applying to More Than One College or University
The ApplyTexas Application was created to help make applying to a Texas institution of higher education
easier. You are strongly encouraged to apply online at www.applytexas.org.
The following procedure is recommended for students who wish to apply to more than one college or
university and who complete a paper application:
1. Each institution has its own unique admission instructions and required credentials. Before mailing
your application, refer to the web site or contact the admissions office of each college or university to
which you apply for additional institutional information.
2. Complete Parts I and II of the ApplyTexas Application. Leave item #17 blank as you are completing
the application.
3. Photocopy the original completed application and make one copy for each college or university to
which you are applying. Keep the original application for your files in the event you wish to apply to
other institutions at a later time.
4. On each copy of the application, complete item #17 by entering the name of the college or to which
you are applying and your first and second choice of major. Follow the instructions for completing
the ApplyTexas Application to ensure proper listing of major(s).
5. Submit one completed application to each college or university to which you are applying, including
Part II, essays and application fee if these items are required by the institution. Not all institutions
require applicants to send in the same parts of the common application. See the general application
information section on pages ii through v of this booklet to determine what items are required by the
institutions to which you apply.

 Notification of Rights under the Family Educational Rights and Privacy Act (FERPA): Information collected about you

private or independent institution of higher education and includes a transfer student, or a student who previously attended an

A student is not required to submit evidence of receiving the vaccination against bacterial meningitis, or a booster dose, if the
student is 30 years of age or older, or if the student is enrolled only in online or other distance education courses.
A student is not required to submit evidence of receiving the vaccination against bacterial meningitis if the student submits to
the institution:
• An affidavit or certificate signed by a physician who is duly registered and licensed to practice medicine in the
United States, stating that in the physician’s opinion, the vaccination would be injurious to the health and well-being
of the student; or
• An affidavit signed by the student stating that the student declines the vaccination for reasons of conscience,
including a religious belief. A conscientious exemption form from the Texas Department of State Health Services
must be used.
If a student has an objection to receiving the vaccination for reasons of conscience, there are several options for students to
receive the conscientious objection forms. A student may submit a request on a DSHS online form, fax, or regular mail. DSHS
has 5 days from the date they receive the requests to process and mail out the form. Unless it’s a very busy time of the year, like
back-to-school in August, the forms are sent out within 24-36 hours. However, it may take a student up to two weeks to receive
the forms. The forms would then need to be completed, notarized and provided to the designated school official at your
