Pre-Employment Requirements
Criminal Background Check: Criminal Background Checks are performed by the Office of Clinical
Education Compliance located in DuBourg Hall, Room 20. Appointments hours are: M-Th 9-4:30.
To make an appointment and complete the registration process use the following link:

that an employee has cleared their cbc, so it is very important to select the first appointment available.
Employee Health Exam: a pre-employment health screen is required for all employees with direct
patient contact or non-direct patient contact who work in SOM, DPS, Union, or positions living in
residence halls. Employment is contingent upon passing the drug screen. Contact Employee Health M-F
7:30-4:30 for an appointment by calling 314-268-5499. Please allow 1 ½ hours for this appointment.
Please be prepared to provide the title of position, name of the recruiter who made your contingent job
offer and if you are holding a direct patient or non-direct patient contact position.
Full-time employees who plan to enroll for medical benefits may qualify for a $30 reduction in employee
health premiums by opting to participate in the University wellness plan called Vitality. In order to qualify
for this reduction, two conditions must be met. The first is to obtain a biometric screening from your
physician or by requesting this screening when you schedule your visit with employee health. It is
necessary to request an early-morning appointment and remember to fast after midnight the night before.
The second condition will be met once you enroll in the Vitality plan and complete an on-line health
assessment. You will be provided instructions about Vitality enrollment at your benefits orientation.
Please share your appointment times for both the CBC and employee health (if applicable) by emailing your recruiter with this information.
Provide Transcripts: If this box is checked, HR was unable to verify your degree using our on-line
software program and it is necessary for you to request the original transcript from the school which you
graduated. The transcript must be provided to the recruiter directly from the institution attended, either by
e-mail or regular postal delivery. Please have transcripts sent to:

Provide Licensure, Certification, or Registration: If this box is checked, please provide HR with a
copy of your current professional authorization.
Complete I-9 Form (Employment Eligibility Verification) You will not be allowed to start work until you
have successfully completed I-9 Form in Human Resources. Saint Louis University is utilizing I-9
eXpress, an online product, to collect and store the University’s I-9 Form. Employees will be required to
complete an online Form I-9 no earlier than 31 days prior to his or her start date and no later than
the first day of employment. After completing the employee section of the Form I-9, you will need to
visit the human resources office, Wool Center, with acceptable form(s) of identification to complete the
process. A list of acceptable forms of identification is provided when the employee has successfully
completed the online Form I-9.
Please follow the steps below to complete the electronic Form I-9 through I-9 eXpress:
1. Use this link:: http://www.newi9.com/
2. In the Employer Name or Code field enter 10065, then click Go.
3. Once on the login page you will be required to enter the text shown, for security validation
purposes, and then click continue.
4. Complete the designated fields.
5. Provide your electronic signature by checking the box and providing your initials.
6. You will be taken to a new page which states you have successfully completed Section 1 of the
Form I-9. Please print this page for your records. This page will provide you with important
information regarding the forms of identification you may provide to your employer for the purpose
of verifying employment eligibility. Bring the indicated forms of identification to Human
Resources, 3545 Lindell Blvd. (Wool Center, St. Louis, MO 63103).

Faculty/Staff Personal Information Form: Please complete the Faculty/Staff Personal Information
Form and bring it with you to human resources on or before your first day of hire.

Complete tax forms. Default tax withholding for Federal and State withholdings is Single with Zero
Allowances. New employees will be setup with those rates of withholding until valid W-4 forms are
received in Payroll Services. You may obtain forms from the front desk in HR or print the forms and return

E-Mail Access and Banner Self-Service Access Instructions: Please use your banner ID to set-up
your e-mail account and gain access to banner self-service, which is used to set-up direct deposit,
submit timesheets, and update other personal information. Your banner ID was included in your offer
letter and is a 9- digit number beginning with zeros.
Go to www.slu.edu/newslink.xml and choose banner self-service from the left margin under Workplace
Tools and follow these instructions:
#1 - Click on the pasword management site, https://password.slu.edu/ and activate/verify your
SLU Net ID. Your initial password has been set to "Id" plus the last 6 digits of your banner ID.
The password management site is used to change your SLU Net ID password which will impact the
following applications:
- Computer Login (Network Drives, MyFiles)
-Google Apps (e-mail)
- mySLU portal
- Self-Service Banner
- Internet Native Banner
- Billiken Secure Connect (VPN)
- Wireless Access (via SLU Net)
- SLU Global
Note: If you are unsuccessful with your initial log-on please wait 48 hours and try again.
#2 Once you have completed the above, please test your access by going directly into mySLU (this is the
way you'll normally access your programs at work) Login to mySLU using your SLU Net ID and new
password. MySLU may be found at https://myslu.slu.edu/cp/home/displaylogin
#3 From mySLU click on the Tools Tab, then click the link to Banner Self-Service. Banner Self-Service is
used to submit time sheets, set-up direct deposit and access employee and personal information.
#4 For E-Mail Access (Google Apps) Go to http://mail.slu.edu and use your same username and new
password to log in. Google Apps may also be found under the Tools Tab on mySLU.
Direct Deposit Required - Employees are required to establish direct deposit in order to receive their
paychecks. New employees are responsible for using banner self-service to set-up their account
information. To set-up your direct deposit account (s) go to https://myslu.slu.edu/cp/home/displaylogin
select banner self-service, then “employee” under the main menu, “pay information” and “direct deposit”
and follow the on-line instructions.
ID badge and Parking Arrangements: Once you complete your tax and I-9 paperwork with the front
desk in HR, you will be issued a temporary ID. Bring this temporary ID to the parking and card services
department, located at 221 N. Grand, DuBourg Hall, 1st Floor, Rm.33 where they will take your picture for
your permanent ID and issue a parking tag for the parking lot on which you plan to park on a daily basis.

New Hire Confirmation Receipt: Please bring this with you to human resources on or before your first
day. Human Resources will confirm this information and you will provide this to your manager or
department administrator. This document must be presented to your manager or department
administrator before you can start working.

Benefit plan selections must be made within thirty-one (31) days of your eligibility date.
Employees in positions covered by collective bargaining agreements must make plan selections
within 31 days following completion of their probationary period.
